Position Summary:
VitalHire is seeking a skilled and compassionate RN - Case Management to join our growing clinician community. This high-impact opportunity offers a 5x8 Days at a leading Hospital in Burbank, CA. Key Responsibilities:
• Perform Care Management duties for acute medical patients with 2+ years experience in Care Coordination and Discharge Planning • Manage patient caseload of up to 25 patients with 1:25 ratio during day shifts • Conduct patient assessments and determine medical necessity per evidence-based guidelines • Coordinate care and discharge planning for adult and geriatric patient populations • Work with HMOs, IPAs, and other managed care organizations • Utilize Epic charting system for computerized documentation • Apply knowledge of CMS guidelines, CPT coding, DRG classifications, and HIPAA compliance • Perform needs assessments and order durable medical equipment (DME) as required • Develop and implement plans of care for patients across various units including ICU, SDU/PCU/IMC/Obs • Provide disease management and care coordination services • Work typically one full weekend per 4 weeks subject to department needs • Maintain current state licensure and required certifications • Complete pre-start and annual training modules averaging 16 hours • Commit to working 3 of 5 major holidays (Thanksgiving, Christmas Eve, Christmas Day, New Year's Eve, New Year's Day) • Float between facilities within the state as required • Use facility-provided communication devices when necessary for assignment duties • Provide minimum of 2 supervisory references covering 1 year of employment in comparable specialty/setting • Self-report time spent on non-billable training modules • Complete 40 hours of orientation training
Job Requirements:
- Active, unencumbered license in CA
- Minimum of 1-7 years of relevant experience, depending on specialty
- Current Certifications: BLS and/or ACLS, plus any specialty-specific requirements
- Experience with EMR systems (Epic, Cerner, as applicable)
Other Required Documentation:
- Current Resume listing detailed work history (MM/YYYY format)
- Two current clinical references
- State-required professional license
- Physicial exam (completed within the past 12 months)
- TB skin test or Quantiferon Gold (within the past 12 months)
- Immunity Titers - MMR/Hep B/Varicella (declinations may be accepted)
- Current Tdap and Flu vaccinations
- Must pass background check and drug screening
