Position Summary:
VitalHire is seeking a skilled and compassionate RN - Case Management to join our growing clinician community. This high-impact opportunity offers a 5x8 Days at a leading Hospital in Torrance, CA. Key Responsibilities:
• Perform acute care case management and discharge planning services
• Maintain current CA state RN license and required certifications
• Utilize Epic charting system for computerized documentation
• Apply InterQual criteria for admission reviews and medical necessity determinations
• Conduct patient assessments and coordinate care across interdisciplinary teams
• Develop and participate in treatment plans and family meetings
• Manage caseload of up to 25 patients with 1:20 daily ratios
• Coordinate discharge planning and arrange DME, SNF, and ambulance transport independently
• Work with various insurance types including HMOs, IPAs, Medicare, and MediCAL
• Handle admission criteria reviews for ED, Mother Baby, and NICU units
• Provide case management for diverse patient populations including neonates through geriatrics
• Work every other weekend and be available for holiday coverage
• Complete 40 hours of orientation and required pre-start modules
• Float between facilities within the state as needed
• Maintain knowledge of Medicare guidelines and evidence-based practices
• Handle complex cases involving adoption/foster care, substance abuse, homelessness, and mental health
• Process appeals, denials, and prior authorizations as needed
• Provide minimum of 2 supervisory references covering 1 year of comparable experience
• Complete required compliance documentation and annual training modules
• Self-report time spent on non-billable training activities
Job Requirements:
- Active, unencumbered license in CA
- Minimum of 1-7 years of relevant experience, depending on specialty
- Current Certifications: BLS and/or ACLS, plus any specialty-specific requirements
- Experience with EMR systems (Epic, Cerner, as applicable)
Other Required Documentation:
- Current Resume listing detailed work history (MM/YYYY format)
- Two current clinical references
- State-required professional license
- Physicial exam (completed within the past 12 months)
- TB skin test or Quantiferon Gold (within the past 12 months)
- Immunity Titers - MMR/Hep B/Varicella (declinations may be accepted)
- Current Tdap and Flu vaccinations
- Must pass background check and drug screening
