Position Summary:
VitalHire is seeking a skilled and compassionate RN - Case Management to join our growing clinician community. This high-impact opportunity offers a 5x8 Days at a leading Hospital in Torrance, CA. Key Responsibilities:
• Perform acute care case management and discharge planning services • Conduct admission reviews using InterQual criteria for ED, Mother Baby and NICU units • Assess patients and coordinate comprehensive care plans • Develop and participate in treatment plans and family meetings • Coordinate discharge planning for patients across multiple units • Arrange durable medical equipment (DME), skilled nursing facility placements, and ambulance transport independently • Work with various insurance types including HMOs, IPAs, Medicare, and MediCAL • Provide case management for diverse patient populations including neonates, pediatrics, adults, and geriatrics • Handle complex cases involving adoption/foster care, substance abuse treatment, homelessness, and mental health crises • Manage advance directives, POLST, and protective services processes • Utilize Epic charting system for computerized documentation • Maintain caseload of up to 25 patients with 1:20 daily ratios • Work every other weekend and cover up to 2 units focusing on discharge planning • Commit to working 3 of 5 major holidays • Complete required pre-start and annual training modules • Float between facilities within the state as required • Maintain current California RN licensure • Provide minimum of 2 supervisory references covering 1 year of comparable experience • Comply with Medicare guidelines and medical necessity determinations • Coordinate with interdisciplinary teams including IV teams, physical therapy, respiratory services, and social services
Job Requirements:
- Active, unencumbered license in CA
- Minimum of 1-7 years of relevant experience, depending on specialty
- Current Certifications: BLS and/or ACLS, plus any specialty-specific requirements
- Experience with EMR systems (Epic, Cerner, as applicable)
Other Required Documentation:
- Current Resume listing detailed work history (MM/YYYY format)
- Two current clinical references
- State-required professional license
- Physicial exam (completed within the past 12 months)
- TB skin test or Quantiferon Gold (within the past 12 months)
- Immunity Titers - MMR/Hep B/Varicella (declinations may be accepted)
- Current Tdap and Flu vaccinations
- Must pass background check and drug screening
