Position Summary:
VitalHire is seeking a skilled and compassionate RN - Case Management to join our growing clinician community. Key Responsibilities:
• Perform case management and care coordination for acute care hospital patients with 1:20 patient ratio
• Conduct discharge planning and assess patients using InterQual admission criteria
• Develop and participate in treatment plans and organize family meetings
• Coordinate care for patients with diverse needs including drug/alcohol abuse, homelessness, mental health issues
• Manage advance directives, POLST, and protective services processes
• Work with HMOs, IPAs, and various insurance providers
• Arrange durable medical equipment (DME), skilled nursing facility (SNF) placements, and ambulance transport independently
• Provide care coordination for adolescents, adults, and geriatric patients
• Collaborate with interdisciplinary team including social services and transportation
• Utilize Epic charting system for computerized documentation
• Maintain proficiency in Microsoft applications
• Work weekend rotation to meet department needs
• Commit to working 3 of 5 major holidays (Thanksgiving, Christmas Eve, Christmas Day, New Year's Eve, New Year's Day)
• Complete required modules pre-start and annually (average 16 hours, non-billable)
• Self-report time spent on training modules with completion recorded in Workramp transcripts
• Float between all facilities within the state as required
• Utilize facility-provided communication devices when required for assignment duties
• Maintain current state licensure and required certifications
• Provide minimum of 2 supervisory references covering 1 year of employment within comparable specialty/setting
• Complete 40-hour orientation program
• Adhere to facility dress code requirements (scrubs or business attire)
Job Requirements:
- Active, unencumbered license in CA
- Minimum of 1-7 years of relevant experience, depending on specialty
- Current Certifications: BLS and/or ACLS, plus any specialty-specific requirements
- Experience with EMR systems (Epic, Cerner, as applicable)
Other Required Documentation:
- Current Resume listing detailed work history (MM/YYYY format)
- Two current clinical references
- State-required professional license
- Physicial exam (completed within the past 12 months)
- TB skin test or Quantiferon Gold (within the past 12 months)
- Immunity Titers - MMR/Hep B/Varicella (declinations may be accepted)
- Current Tdap and Flu vaccinations
- Must pass background check and drug screening
