Position Summary:
VitalHire is seeking a skilled and compassionate RN - Case Management to join our growing clinician community. Key Responsibilities:
• Perform case management services with 2+ years acute care hospital experience in care coordination and discharge planning
• Maintain 1:20 patient ratio while managing adolescent, adult, and geriatric patient populations
• Utilize Epic charting system for computerized documentation
• Apply InterQual admission criteria for patient assessments
• Coordinate care and develop discharge planning for patients with diverse medical needs
• Assess patients experiencing homelessness, mental health crises, and substance abuse issues
• Arrange durable medical equipment (DME), skilled nursing facility (SNF) placements, and ambulance transport independently
• Work with HMOs, IPAs, and various insurance systems
• Organize and participate in interdisciplinary family meetings
• Participate in treatment plan development with healthcare team
• Collaborate with social services and transportation departments
• Manage advance directives, POLST forms, and protective services processes
• Work weekend rotations and 3 out of 5 major holidays as required
• Complete 40-hour orientation and required pre-start modules
• Float between facilities within the state as assigned
• Maintain current CA state RN licensure
• Provide minimum of 2 supervisory references covering 1 year of comparable specialty experience
• Complete annual compliance modules and training requirements
• Utilize facility-provided communication devices when required
• Self-report time spent on non-billable training modules
Job Requirements:
- Active, unencumbered license in CA
- Minimum of 1-7 years of relevant experience, depending on specialty
- Current Certifications: BLS and/or ACLS, plus any specialty-specific requirements
- Experience with EMR systems (Epic, Cerner, as applicable)
Other Required Documentation:
- Current Resume listing detailed work history (MM/YYYY format)
- Two current clinical references
- State-required professional license
- Physicial exam (completed within the past 12 months)
- TB skin test or Quantiferon Gold (within the past 12 months)
- Immunity Titers - MMR/Hep B/Varicella (declinations may be accepted)
- Current Tdap and Flu vaccinations
- Must pass background check and drug screening
