Position Summary:
VitalHire is seeking a skilled and compassionate Social Worker - Licensed Master Social Worker to join our growing clinician community. Key Responsibilities:
• Provide psychosocial and emotional support to patients and families in the Emergency Department • Complete psychosocial assessments and identify appropriate discharge plans from a person-centered approach • Conduct biopsychosocial assessments and develop comprehensive care plans • Provide brief clinical interventions and crisis intervention services as appropriate • Assist patients with resource navigation and coordination of care • Screen for suicidality and engage in safety planning when appropriate • Complete mandatory reports of harm based on state statutes • Coordinate with outside entities and community resources • Work collaboratively with RN Case Managers and interdisciplinary care teams • Facilitate discharge planning and contingency planning • Document care delivery, plans, goals, and patient responses according to established protocols • Educate patients and caregivers to facilitate safe discharge transitions • Participate in care conference activities and interdisciplinary rounds • Float within the hospital or to affiliate hospitals as needed • Work weekend shifts and 2 out of 3 major holidays (Thanksgiving, Christmas, New Year) • Utilize Epic charting system for all documentation • Maintain current Alaska state licensure as LMSW or LCSW • Complete required pre-start and annual training modules • Provide minimum of 2 supervisory references covering 1 year of employment in comparable setting • Adhere to NASW Code of Ethics and facility policies and procedures
Job Requirements:
- Active, unencumbered license in AK
- Minimum of 1-7 years of relevant experience, depending on specialty
- Current Certifications: BLS and/or ACLS, plus any specialty-specific requirements
- Experience with EMR systems (Epic, Cerner, as applicable)
Other Required Documentation:
- Current Resume listing detailed work history (MM/YYYY format)
- Two current clinical references
- State-required professional license
- Physicial exam (completed within the past 12 months)
- TB skin test or Quantiferon Gold (within the past 12 months)
- Immunity Titers - MMR/Hep B/Varicella (declinations may be accepted)
- Current Tdap and Flu vaccinations
- Must pass background check and drug screening
