Position Summary:
VitalHire is seeking a skilled and compassionate OT - Therapy Rehab to join our growing clinician community. This high-impact opportunity offers a 5x8 Days at a leading Rehab in Anchorage, AK. Key Responsibilities:
• Provide occupational therapy services in home health setting • Complete OASIS documentation for Start of Care visits • Visit 4-6 patients per day maintaining 27-30 productivity points standard • Maintain current BLS and NBCOT certifications • Utilize HomeCare HomeBase charting system for documentation • Provide timely communication and documentation as required • Travel up to 100 mile radius within assigned territory using personal transportation • Complete required compliance documentation within 3 weeks of start • Work 8-hour day shifts without weekend or call requirements • Maintain valid Alaska occupational therapy license • Provide care for adult patients in home health setting • Complete required training modules and hospice/home health checklist • Participate in on-call rotation as assigned • Return all provided documentation tools upon completion of assignment • Submit requested time off at time of application • Demonstrate proficiency in various OT skills including ADLs, splinting, prosthetics, and rehabilitation techniques
Job Requirements:
- Active, unencumbered license in AK
- Minimum of 1-7 years of relevant experience, depending on specialty
- Current Certifications: BLS and/or ACLS, plus any specialty-specific requirements
- Experience with EMR systems (Epic, Cerner, as applicable)
Other Required Documentation:
- Current Resume listing detailed work history (MM/YYYY format)
- Two current clinical references
- State-required professional license
- Physicial exam (completed within the past 12 months)
- TB skin test or Quantiferon Gold (within the past 12 months)
- Immunity Titers - MMR/Hep B/Varicella (declinations may be accepted)
- Current Tdap and Flu vaccinations
- Must pass background check and drug screening
