Position Summary:
VitalHire is seeking a skilled and compassionate RN - Case Management to join our growing clinician community. This high-impact opportunity offers a 5x8 Days at a leading Hospital in Burbank, CA. Key Responsibilities:
• Provide acute medical social work services performing both social work and discharge planning roles
• Manage caseload of up to 25 patients with 1:25 ratio during day shifts
• Utilize Epic charting system for documentation and patient records
• Conduct care coordination and collaborative care activities
• Develop comprehensive plans of care to meet patient and caregiver needs
• Perform discharge planning and needs assessments
• Order durable medical equipment (DME) as needed
• Participate in care conference activities and family meetings
• Organize and facilitate family meetings as required
• Participate in the development of treatment plans
• Provide services for adult and geriatric patient populations
• Address complex cases involving drug/alcohol abuse treatment options
• Assist patients experiencing homelessness
• Support patients with mental health history or in crisis situations
• Handle advance directive and POLST documentation
• Navigate protective services processes when needed
• Work typically one full weekend per 4 weeks subject to department needs
• Commit to working 3 of the 5 major holidays
• Float between facilities within the state as required
• Complete required modules pre-start and annually
• Maintain current state licensure
• Provide minimum of 2 supervisory references covering 1 year of employment within comparable specialty/setting
• Complete 40 hours of orientation
• Self-report time spent on non-billable training modules
• Utilize facility-provided communication devices when required
Job Requirements:
- Active, unencumbered license in CA
- Minimum of 1-7 years of relevant experience, depending on specialty
- Current Certifications: BLS and/or ACLS, plus any specialty-specific requirements
- Experience with EMR systems (Epic, Cerner, as applicable)
Other Required Documentation:
- Current Resume listing detailed work history (MM/YYYY format)
- Two current clinical references
- State-required professional license
- Physicial exam (completed within the past 12 months)
- TB skin test or Quantiferon Gold (within the past 12 months)
- Immunity Titers - MMR/Hep B/Varicella (declinations may be accepted)
- Current Tdap and Flu vaccinations
- Must pass background check and drug screening
