Position Summary:
VitalHire is seeking a skilled and compassionate RN - Case Management to join our growing clinician community. This high-impact opportunity offers a 5x8 Days at a leading Hospital in Tarzana, CA. Key Responsibilities:
• Provide case management services for acute care medical hospital patients with 1:35 patient ratio
• Conduct biopsychosocial assessments and collect/analyze patient information
• Coordinate care and services in a cost-effective manner with internal and external team members
• Document care delivery, plans, goals, and patient/caregiver teaching according to plan of care
• Educate patients and caregivers to facilitate discharge planning
• Coordinate services to home care and equipment following discharge
• Refer patients to other disciplines, resources, rehabilitation and long-term care facilities
• Participate in care conference activities and collaborative care planning
• Assist patients with navigating healthcare systems and coverage criteria
• Provide high-risk intervention for patient vulnerability or safety concerns
• Address cultural issues and work with patients experiencing communication problems
• Handle cases involving adoption/foster care, drug/alcohol abuse treatment, homelessness, and mental health crises
• Manage advance directives, POLST, and protective services processes
• Utilize Epic charting system for documentation and care coordination
• Work with adult and geriatric patient populations
• Complete required orientation hours and annual training modules
• Work every other weekend and commit to 3 of 5 major holidays
• Float between facilities within the state as required
• Maintain minimum of 2 supervisory references covering 1 year of employment
• Complete skills checklist and compliance documentation
• Use facility-provided communication devices when required for patient care duties
Job Requirements:
- Active, unencumbered license in CA
- Minimum of 1-7 years of relevant experience, depending on specialty
- Current Certifications: BLS and/or ACLS, plus any specialty-specific requirements
- Experience with EMR systems (Epic, Cerner, as applicable)
Other Required Documentation:
- Current Resume listing detailed work history (MM/YYYY format)
- Two current clinical references
- State-required professional license
- Physicial exam (completed within the past 12 months)
- TB skin test or Quantiferon Gold (within the past 12 months)
- Immunity Titers - MMR/Hep B/Varicella (declinations may be accepted)
- Current Tdap and Flu vaccinations
- Must pass background check and drug screening
