Position Summary:
VitalHire is seeking a skilled and compassionate RN - Case Management to join our growing clinician community. Key Responsibilities:
• Perform case management services with 2+ years of acute care hospital experience in care coordination and discharge planning • Maintain patient ratios of 1:20 during day shifts • Utilize Epic charting system for computerized documentation • Work weekend rotation to meet department needs • Maintain valid CA state RN license upon start date • Complete 40 hours of orientation training • Apply InterQual admission criteria for patient assessments • Coordinate care for adolescents, adults, and geriatric patients • Collaborate with interdisciplinary support teams including social services and transportation • Assess patients with complex needs including drug/alcohol abuse, homelessness, and mental health crises • Organize and participate in family meetings • Participate in treatment plan development • Work with HMOs and IPAs for insurance coordination • Arrange durable medical equipment, skilled nursing facility placements, and ambulance transport independently • Manage advance directives, POLST forms, and protective services processes • Demonstrate proficiency in Microsoft applications • Float between all facilities within the state as required • Work 3 of 5 major holidays (Thanksgiving, Christmas Eve, Christmas Day, New Year's Eve, New Year's Day) • Complete pre-start and annual training modules • Provide minimum of 2 supervisory references covering 1 year of employment in comparable specialty • Self-report time spent on non-billable training modules • Use facility-provided communication devices when required for assignment duties
Job Requirements:
- Active, unencumbered license in CA
- Minimum of 1-7 years of relevant experience, depending on specialty
- Current Certifications: BLS and/or ACLS, plus any specialty-specific requirements
- Experience with EMR systems (Epic, Cerner, as applicable)
Other Required Documentation:
- Current Resume listing detailed work history (MM/YYYY format)
- Two current clinical references
- State-required professional license
- Physicial exam (completed within the past 12 months)
- TB skin test or Quantiferon Gold (within the past 12 months)
- Immunity Titers - MMR/Hep B/Varicella (declinations may be accepted)
- Current Tdap and Flu vaccinations
- Must pass background check and drug screening
