Position Summary:
VitalHire is seeking a skilled and compassionate LPN/LVN - LPN/LVN to join our growing clinician community. This high-impact opportunity offers a 5x8 Days at a leading Hospital in Tarzana, CA. Key Responsibilities:
• Manage case workload of up to 50 discharges, with higher numbers over weekends
• Provide case management services in acute care hospital setting for adult patients
• Perform discharge planning and care coordination
• Conduct admission assessments and determine medical necessity per evidence-based guidelines
• Handle appeals and denials processes
• Complete prior authorizations and needs assessments for durable medical equipment (DME)
• Develop and implement plans of care for patients
• Utilize Epic charting system for documentation and case management
• Collaborate with interdisciplinary team including physical therapy, social services, transportation, and unit secretary
• Ensure compliance with CMS guidelines and regulations
• Adhere to HIPAA guidelines for patient privacy and confidentiality
• Follow The Joint Commission core measures and national safety goals
• Work with various units including ICU, SDU/PCU/IMC/Obs, and surgical services
• Handle worker's compensation case management as needed
• Work every other weekend as required
• Commit to working 3 of the 5 major holidays (Thanksgiving, Christmas Eve, Christmas Day, New Year's Eve, New Year's Day)
• Complete required pre-start and annual training modules
• Float between facilities within the state as assigned
• Maintain valid California state licensure
• Complete 40 hours of orientation training
• Use facility-provided communication devices when required for assignment duties
• Self-report time spent on non-billable training modules
Job Requirements:
- Active, unencumbered license in CA
- Minimum of 1-7 years of relevant experience, depending on specialty
- Current Certifications: BLS and/or ACLS, plus any specialty-specific requirements
- Experience with EMR systems (Epic, Cerner, as applicable)
Other Required Documentation:
- Current Resume listing detailed work history (MM/YYYY format)
- Two current clinical references
- State-required professional license
- Physicial exam (completed within the past 12 months)
- TB skin test or Quantiferon Gold (within the past 12 months)
- Immunity Titers - MMR/Hep B/Varicella (declinations may be accepted)
- Current Tdap and Flu vaccinations
- Must pass background check and drug screening
