Position Summary:
VitalHire is seeking a skilled and compassionate LPN/LVN - LPN/LVN to join our growing clinician community. This high-impact opportunity offers a 5x8 Days at a leading Hospital in Tarzana, CA. Key Responsibilities:
• Manage case work load of up to 50 discharges, with higher numbers over weekends • Perform case management duties in adult acute care hospital setting • Coordinate care with interdisciplinary team including Physical Therapy, Social Services, Transportation, and Unit Secretary • Utilize Epic charting system for documentation and patient records • Conduct discharge planning and care coordination for adult patients • Perform admission criteria assessments and needs assessments • Handle appeals and denials processes • Determine medical necessity per evidence-based guidelines • Process prior authorizations as needed • Order durable medical equipment (DME) when required • Develop and maintain plans of care for patients • Work with DRG (Diagnosis Related Groups) classifications • Ensure compliance with HIPAA guidelines • Adhere to The Joint Commission core measures and national safety goals • Handle workers compensation case management when applicable • Work with CMS (Centers for Medicare and Medicaid Services) requirements • Provide coverage for ICU, SDU/PCU/IMC/Obs, and Surgical Services units • Complete required pre-start and annual training modules • Commit to working 3 of the 5 major holidays (Thanksgiving, Christmas Eve, Christmas Day, New Year's Eve, New Year's Day) • Float between facilities within the state as required • Provide minimum of 2 supervisory references covering 1 year of employment within comparable specialty/setting • Maintain required certifications and skills competencies • Complete skills checklist and compliance documentation • Utilize facility-provided communication devices when required for assignment duties
Job Requirements:
- Active, unencumbered license in CA
- Minimum of 1-7 years of relevant experience, depending on specialty
- Current Certifications: BLS and/or ACLS, plus any specialty-specific requirements
- Experience with EMR systems (Epic, Cerner, as applicable)
Other Required Documentation:
- Current Resume listing detailed work history (MM/YYYY format)
- Two current clinical references
- State-required professional license
- Physicial exam (completed within the past 12 months)
- TB skin test or Quantiferon Gold (within the past 12 months)
- Immunity Titers - MMR/Hep B/Varicella (declinations may be accepted)
- Current Tdap and Flu vaccinations
- Must pass background check and drug screening
