Position Summary:
VitalHire is seeking a skilled and compassionate PT - Physical Therapist to join our growing clinician community. This high-impact opportunity offers a 5x8 Days at a leading Rehab in Portland, OR. Key Responsibilities:
• Provide physical therapy services in home health setting with up to 6 patient visits per day
• Maintain 30 productivity points standard and meet point expectations
• Perform comprehensive patient assessments and evaluations
• Develop and implement individualized treatment plans and home exercise programs
• Complete OASIS documentation requirements
• Utilize Epic Remote Client for Home Health charting and Rover phone app
• Provide patient and family education on therapeutic interventions
• Perform strength testing, gait training, and mobility assessments
• Administer therapeutic modalities including manual therapy, wound care, and assistive device training
• Manage patients with various diagnoses including post-stroke, orthopedic post-op, Parkinson's, ALS, MS, and dementia
• Schedule patient appointments and order necessary supplies
• Perform lifting, transfers, and mobility training with patients
• Travel to patient homes within 60-mile radius using personal transportation
• Maintain current BCLS certification and complete required modules pre-start and annually
• Provide minimum of 2 supervisory references covering 1 year of comparable specialty experience
• Complete required compliance documentation and non-billable training modules
• Self-report time spent on training modules and maintain accurate documentation
• Utilize facility-provided communication devices when required for patient care coordination
Job Requirements:
- Active, unencumbered license in OR
- Minimum of 1-7 years of relevant experience, depending on specialty
- Current Certifications: BLS and/or ACLS, plus any specialty-specific requirements
- Experience with EMR systems (Epic, Cerner, as applicable)
Other Required Documentation:
- Current Resume listing detailed work history (MM/YYYY format)
- Two current clinical references
- State-required professional license
- Physicial exam (completed within the past 12 months)
- TB skin test or Quantiferon Gold (within the past 12 months)
- Immunity Titers - MMR/Hep B/Varicella (declinations may be accepted)
- Current Tdap and Flu vaccinations
- Must pass background check and drug screening
