Position Summary:
VitalHire is seeking a skilled and compassionate PT - Physical Therapist to join our growing clinician community. This high-impact opportunity offers a 5x8 Days at a leading Rehab in Anchorage, AK. Key Responsibilities:
• Provide skilled physical therapy services in patients' homes as part of home health care team
• Conduct patient assessments and develop individualized treatment plans
• Perform OASIS documentation and complete start of care evaluations
• Visit 4-6 patients per day maintaining 27-30 productivity points standard
• Provide treatment for typical home health diagnoses including wounds, weakness, falls, post-operative care, and mobility issues
• Educate patients and families on home exercise programs and safety measures
• Manage and troubleshoot durable medical equipment issues in the home setting
• Utilize Home Care Home Base charting system for timely documentation
• Maintain current BLS certification and valid state physical therapy license
• Provide skilled intermittent care including lifting, transfers, and patient transport assistance
• Schedule appointments and maintain effective telephone communication with patients and healthcare team
• Travel within assigned territory with potential radius up to 100 miles
• Participate in on-call rotation as required
• Complete required compliance documentation and training programs
• Maintain own reliable transportation and valid driver's license
• Adhere to safety and injury prevention protocols in home environments
• Collaborate with interdisciplinary team members and utilize interpretation services when needed
Job Requirements:
- Active, unencumbered license in AK
- Minimum of 1-7 years of relevant experience, depending on specialty
- Current Certifications: BLS and/or ACLS, plus any specialty-specific requirements
- Experience with EMR systems (Epic, Cerner, as applicable)
Other Required Documentation:
- Current Resume listing detailed work history (MM/YYYY format)
- Two current clinical references
- State-required professional license
- Physicial exam (completed within the past 12 months)
- TB skin test or Quantiferon Gold (within the past 12 months)
- Immunity Titers - MMR/Hep B/Varicella (declinations may be accepted)
- Current Tdap and Flu vaccinations
- Must pass background check and drug screening
