Position Summary:
VitalHire is seeking a skilled and compassionate PT - Physical Therapist to join our growing clinician community. This high-impact opportunity offers a 5x8 Days at a leading Rehab in Anchorage, AK. Key Responsibilities:
• Provide physical therapy services in home health setting to adult and geriatric patients
• Conduct 4-6 patient visits per day maintaining 27-30 productivity points standard
• Complete OASIS documentation as required for Start of Care visits
• Utilize Home Care Home Base (HCHB) charting system for documentation
• Provide skilled intermittent physical therapy care in patients' homes
• Follow treatment plans and home exercise programs established by supervising PT
• Troubleshoot durable medical equipment issues in patient homes
• Treat typical home health diagnoses including wounds, weakness, falls, post-operative conditions
• Provide patient and family education on treatment plans and safety measures
• Perform lifting, transfers, and patient transport as needed
• Schedule appointments and maintain timely communication with patients and team
• Complete documentation in timely manner according to facility requirements
• Travel within assigned territory with occasional radius up to 100 miles
• Maintain current BLS certification and state licensure
• Participate in on-call rotation as required
• Complete required compliance documentation and training modules
• Maintain professional appearance in scrubs of any color
• Return all provided documentation tools and equipment at assignment completion
Job Requirements:
- Active, unencumbered license in AK
- Minimum of 1-7 years of relevant experience, depending on specialty
- Current Certifications: BLS and/or ACLS, plus any specialty-specific requirements
- Experience with EMR systems (Epic, Cerner, as applicable)
Other Required Documentation:
- Current Resume listing detailed work history (MM/YYYY format)
- Two current clinical references
- State-required professional license
- Physicial exam (completed within the past 12 months)
- TB skin test or Quantiferon Gold (within the past 12 months)
- Immunity Titers - MMR/Hep B/Varicella (declinations may be accepted)
- Current Tdap and Flu vaccinations
- Must pass background check and drug screening
