Position Summary:
VitalHire is seeking a skilled and compassionate Social Worker - Licensed Social Worker to join our growing clinician community. Key Responsibilities:
• Perform duties as a Licensed Social Worker in short term acute care setting • Maintain current LCSW certification and valid state licensure • Provide social work services with patient ratio of 20:1 • Complete admission criteria and assessment processes • Conduct initial evaluations and develop comprehensive plans of care • Facilitate discharge planning and coordinate transfers to physician appointments • Arrange and coordinate services for home care and equipment following discharge • Complete all required paperwork and accountability reports • Document care delivery, plans, goals, and patient responses according to established protocols • Educate patients and caregivers to facilitate successful discharge outcomes • Participate in care conference activities and interdisciplinary team meetings • Collaborate with internal and external team members for comprehensive care coordination • Work with diverse patient populations including adults with various medical conditions • Handle cases involving adult protective services, guardianship, and emergency detention when required • Coordinate with Medicare, Medicaid, and other benefit programs • Provide crisis intervention for high-risk patients with vulnerability or safety concerns • Float between sister facilities and affiliate hospitals as assigned • Utilize Epic charting system for all documentation requirements • Work weekend shifts and block schedules as required • Commit to working 3 out of 5 major holidays (Thanksgiving, Christmas Eve, Christmas Day, New Year's Eve, New Year's Day) • Complete required pre-start and annual training modules • Maintain transportation to facilitate patient appointments and discharge arrangements • Adhere to NASW Code of Ethics and professional standards • Self-report time spent on non-billable training modules
Job Requirements:
- Active, unencumbered license in OR
- Minimum of 1-7 years of relevant experience, depending on specialty
- Current Certifications: BLS and/or ACLS, plus any specialty-specific requirements
- Experience with EMR systems (Epic, Cerner, as applicable)
Other Required Documentation:
- Current Resume listing detailed work history (MM/YYYY format)
- Two current clinical references
- State-required professional license
- Physicial exam (completed within the past 12 months)
- TB skin test or Quantiferon Gold (within the past 12 months)
- Immunity Titers - MMR/Hep B/Varicella (declinations may be accepted)
- Current Tdap and Flu vaccinations
- Must pass background check and drug screening
