Position Summary:
VitalHire is seeking a skilled and compassionate OT - Therapy Rehab to join our growing clinician community. This high-impact opportunity offers a 5x8 Days at a leading Rehab in Portland, OR. Key Responsibilities:
• Provide occupational therapy services in home health setting
• Conduct patient evaluations and assessments
• Develop and implement plans of care for patients
• Perform reassessments and update treatment plans as needed
• Provide patient and family education
• Complete discharge planning and pre-discharge planning
• Document using EPIC Remote Client for Home Health and Rover phone app
• Complete OASIS documentation requirements
• Maintain BCLS certification
• Treat adult and geriatric patients with various diagnoses including dementia, Parkinson's, ALS, MS, orthopedic post-op, and post-stroke conditions
• Conduct home assessments and safety evaluations
• Provide adaptive equipment training and wheelchair evaluation/positioning
• Perform splinting services including functional, dynamic, static, and serial/inhibitory splints
• Deliver rehabilitation services for upper and lower extremity conditions
• Provide prosthetic training for above knee, below knee, and upper extremity prosthetics
• Conduct driver evaluation and education when required
• Perform joint mobilization and myofascial release techniques
• Complete required pre-start and annual training modules
• Meet productivity standards of approximately 30 points per day
• Visit up to 6 patients per day maintaining 1:1 ratios
• Provide minimum of 2 supervisory references covering 1 year of employment
• Self-report time spent on non-billable training modules
• Float between facilities within the state as required
Job Requirements:
- Active, unencumbered license in OR
- Minimum of 1-7 years of relevant experience, depending on specialty
- Current Certifications: BLS and/or ACLS, plus any specialty-specific requirements
- Experience with EMR systems (Epic, Cerner, as applicable)
Other Required Documentation:
- Current Resume listing detailed work history (MM/YYYY format)
- Two current clinical references
- State-required professional license
- Physicial exam (completed within the past 12 months)
- TB skin test or Quantiferon Gold (within the past 12 months)
- Immunity Titers - MMR/Hep B/Varicella (declinations may be accepted)
- Current Tdap and Flu vaccinations
- Must pass background check and drug screening
