Position Summary:
VitalHire is seeking a skilled and compassionate RN - Home Health to join our growing clinician community. This high-impact opportunity offers a 5x8 Days at a leading SNF in Portland, OR. Key Responsibilities:
• Provide skilled home health nursing care to adult and geriatric patients
• Visit up to 6 patients per day meeting 30 productivity points standard
• Perform admission assessments and start of care documentation
• Conduct routine visits, recertification, and resumption of care
• Manage wound care and wound V.A.C systems
• Provide colostomy/ileostomy management and care
• Administer G-tube site care and management
• Perform medication administration, assessment, and management
• Conduct intravenous infusions and manage PCA pumps
• Administer intradermal, intramuscular, and subcutaneous injections
• Perform pain assessment and management using various pain scales
• Handle discharge planning including safety and non-compliance situations
• Complete Oasis documentation and assessments
• Chart using EPIC Remote Client for Home Health and Rover phone app
• Maintain infection control precautions during patient visits
• Provide disease management and patient education
• Manage PICC lines and catheter care with patient education
• Complete required modules and training programs pre-start and annually
• Maintain current BLS certification
• Float between facilities within the state as required
• Use facility-provided communication devices when required
• Self-report time spent on non-billable training modules
• Provide minimum of 2 supervisory references covering 1 year of employment
• Maintain valid state licensure and COVID vaccination requirements
Job Requirements:
- Active, unencumbered license in OR
- Minimum of 1-7 years of relevant experience, depending on specialty
- Current Certifications: BLS and/or ACLS, plus any specialty-specific requirements
- Experience with EMR systems (Epic, Cerner, as applicable)
Other Required Documentation:
- Current Resume listing detailed work history (MM/YYYY format)
- Two current clinical references
- State-required professional license
- Physicial exam (completed within the past 12 months)
- TB skin test or Quantiferon Gold (within the past 12 months)
- Immunity Titers - MMR/Hep B/Varicella (declinations may be accepted)
- Current Tdap and Flu vaccinations
- Must pass background check and drug screening
