Position Summary:
VitalHire is seeking a skilled and compassionate PTA - Physical Therapist Assistant Rehab to join our growing clinician community. This high-impact opportunity offers a 5x8 Days at a leading Rehab in Walla Walla, WA. Key Responsibilities:
• Provide physical therapy assistant services in home health setting
• Treat 4-5 patients per day with up to 6 patient visits maximum
• Maintain 27-30 productivity points standard based on assigned territory
• Complete timely communication and documentation requirements
• Utilize charting system and documentation tools provided
• Travel within assigned territory with occasional maximum radius of 100 miles
• Provide own transportation for patient visits
• Maintain current BLS certification
• Hold valid WA license or compact license
• Complete compliance requirements within 3-week timeframe
• Work 8-hour day shifts without weekend or call requirements
• Complete OASIS documentation for Start of Care visits
• Utilize HCHB (Homecare Homebase) system
• Submit Requested Time Off at time of application
• Participate in on-call rotation as required
• Complete mandatory Hospice/Home Health Checklist
• Return all documentation tools and equipment upon completion
• Maintain active driver's license throughout assignment
Job Requirements:
- Active, unencumbered license in WA
- Minimum of 1-7 years of relevant experience, depending on specialty
- Current Certifications: BLS and/or ACLS, plus any specialty-specific requirements
- Experience with EMR systems (Epic, Cerner, as applicable)
Other Required Documentation:
- Current Resume listing detailed work history (MM/YYYY format)
- Two current clinical references
- State-required professional license
- Physicial exam (completed within the past 12 months)
- TB skin test or Quantiferon Gold (within the past 12 months)
- Immunity Titers - MMR/Hep B/Varicella (declinations may be accepted)
- Current Tdap and Flu vaccinations
- Must pass background check and drug screening
