Position Summary:
VitalHire is seeking a skilled and compassionate PTA - Physical Therapist Assistant Rehab to join our growing clinician community. Key Responsibilities:
• Provide physical therapy assistant services in home health setting
• Visit 4-6 patients per day maintaining 27-30 productivity points based on territory
• Complete Start of Care (SOC) documentation with OASIS experience
• Maintain timely communication and documentation requirements
• Utilize charting systems and documentation tools provided by facility
• Travel to patient homes within assigned territory with occasional radius up to 100 miles
• Provide skilled PTA services for various conditions including post-operative care, joint replacements, neurological conditions, and rehabilitation
• Work with patients requiring assistive devices such as walkers, crutches, prosthetics, and mobility aids
• Maintain current BLS certification and valid state licensure
• Complete compliance documentation and required training modules
• Participate in on-call rotation as assigned
• Submit requested time off at time of application
• Complete Hospice/Home Health Checklist as mandatory requirement
• Provide own transportation and maintain active driver's license
• Return all documentation tools and equipment provided by facility
Job Requirements:
- Active, unencumbered license in WA
- Minimum of 1-7 years of relevant experience, depending on specialty
- Current Certifications: BLS and/or ACLS, plus any specialty-specific requirements
- Experience with EMR systems (Epic, Cerner, as applicable)
Other Required Documentation:
- Current Resume listing detailed work history (MM/YYYY format)
- Two current clinical references
- State-required professional license
- Physicial exam (completed within the past 12 months)
- TB skin test or Quantiferon Gold (within the past 12 months)
- Immunity Titers - MMR/Hep B/Varicella (declinations may be accepted)
- Current Tdap and Flu vaccinations
- Must pass background check and drug screening
