Position Summary:
VitalHire is seeking a skilled and compassionate PT - Physical Therapist to join our growing clinician community. This high-impact opportunity offers a 5x8 Days at a leading SNF in Walla Walla, WA. Key Responsibilities:
• Provide physical therapy services in home health setting with minimum 1 year recent experience
• Complete 6 daily visits or minimum of 30 points weekly for productivity requirements
• Manage caseload of 5-7 patients per day on average
• Utilize HCHB (Homecare Homebase) EMR system for documentation and patient management
• Complete OASIS assessments for Start of Care (SOC) patients
• Provide care for geriatric patient population with various diagnoses
• Travel up to 60-mile radius from office hub, occasionally more based on patient needs
• Maintain flexibility with assigned territory coverage
• Ensure timely documentation and communication as mandatory requirement
• Participate in on-call rotation as required
• Maintain current BLS certification and valid state physical therapy license
• Complete Hospice/Home Health Checklist as mandatory requirement
• Maintain active driver's license and provide own personal transportation
• Adhere to dress code requiring scrubs of any color (no jeans permitted)
• Work 40 guaranteed hours per week on day shift schedule (08:00 - 16:30)
• Submit Requested Time Off (RTO) at time of application
• Use facility-provided iPad for documentation and patient care activities
Job Requirements:
- Active, unencumbered license in WA
- Minimum of 1-7 years of relevant experience, depending on specialty
- Current Certifications: BLS and/or ACLS, plus any specialty-specific requirements
- Experience with EMR systems (Epic, Cerner, as applicable)
Other Required Documentation:
- Current Resume listing detailed work history (MM/YYYY format)
- Two current clinical references
- State-required professional license
- Physicial exam (completed within the past 12 months)
- TB skin test or Quantiferon Gold (within the past 12 months)
- Immunity Titers - MMR/Hep B/Varicella (declinations may be accepted)
- Current Tdap and Flu vaccinations
- Must pass background check and drug screening
