Position Summary:
VitalHire is seeking a skilled and compassionate PT - Physical Therapist to join our growing clinician community. This high-impact opportunity offers a 5x8 Days at a leading LTC in Walla Walla, WA. Key Responsibilities:
• Provide physical therapy services in home health setting
• Complete 6 daily visits or minimum of 30 points weekly for productivity
• Manage average of 5-7 patients per day
• Work within up to 60-mile radius territory from office hub
• Maintain flexibility with territory assignments based on patient needs
• Provide timely documentation and communication
• Complete OASIS assessments for Start of Care (SOC)
• Utilize HCHB (Homecare Homebase) electronic medical record system
• Maintain current BLS certification
• Maintain valid state physical therapy license
• Provide care for geriatric patient population
• Treat various conditions including post-operative cases, neurological conditions, and orthopedic injuries
• Develop and implement home exercise programs
• Provide patient and family education
• Participate in on-call rotation as required
• Complete required compliance documentation and hospice/home health checklist
• Adhere to professional dress code requirements
• Maintain personal transportation and valid driver's license for patient visits
Job Requirements:
- Active, unencumbered license in WA
- Minimum of 1-7 years of relevant experience, depending on specialty
- Current Certifications: BLS and/or ACLS, plus any specialty-specific requirements
- Experience with EMR systems (Epic, Cerner, as applicable)
Other Required Documentation:
- Current Resume listing detailed work history (MM/YYYY format)
- Two current clinical references
- State-required professional license
- Physicial exam (completed within the past 12 months)
- TB skin test or Quantiferon Gold (within the past 12 months)
- Immunity Titers - MMR/Hep B/Varicella (declinations may be accepted)
- Current Tdap and Flu vaccinations
- Must pass background check and drug screening
