Position Summary:
VitalHire is seeking a skilled and compassionate RN - Case Management to join our growing clinician community. This high-impact opportunity offers a 5x8 Days at a leading Hospital in Burbank, CA. Key Responsibilities:
• Perform acute medical Social Work experience in both Social work and discharge planning roles • Develop plan of care to meet patient/caregiver needs • Participate in collaborative care activities • Engage in care conference activities • Coordinate care for patients • Conduct discharge planning services • Perform needs assessment and order DME • Work with patients experiencing drug/alcohol abuse, homelessness, mental health history or crisis • Handle advance directive and POLST documentation • Navigate protective services processes • Organize and participate in family meetings • Participate in the development of treatment plans • Provide care for adult and geriatric patient populations • Manage caseload of up to 25 patients • Utilize Epic charting system for documentation • Maintain current state licensure • Complete required modules and training programs pre-start and annually • Float between facilities within the state as assigned • Work typically one full weekend per 4 weeks, subject to department need • Commit to working 3 of the 5 major holidays • Provide minimum of 2 supervisory references covering 1 year of employment within comparable specialty/setting • Complete skills checklist and required compliance documentation • Self-report time spent on non-billable training modules • Use facility-provided communication devices when required for assignment duties
Job Requirements:
- Active, unencumbered license in CA
- Minimum of 1-7 years of relevant experience, depending on specialty
- Current Certifications: BLS and/or ACLS, plus any specialty-specific requirements
- Experience with EMR systems (Epic, Cerner, as applicable)
Other Required Documentation:
- Current Resume listing detailed work history (MM/YYYY format)
- Two current clinical references
- State-required professional license
- Physicial exam (completed within the past 12 months)
- TB skin test or Quantiferon Gold (within the past 12 months)
- Immunity Titers - MMR/Hep B/Varicella (declinations may be accepted)
- Current Tdap and Flu vaccinations
- Must pass background check and drug screening
