Position Summary:
VitalHire is seeking a skilled and compassionate RN - Case Management to join our growing clinician community. This high-impact opportunity offers a 5x8 Days at a leading Hospital in Torrance, CA. Key Responsibilities:
• Perform acute care case management and discharge planning for patient caseload up to 25 • Conduct admission criteria assessments using InterQual guidelines for ED, Mother Baby and NICU • Coordinate patient care across multiple disciplines and services • Develop and participate in treatment plans and family meetings • Assess patients with diverse diagnoses including mental health, substance abuse, and complex medical conditions • Arrange discharge planning services including DME, SNF placement, and ambulance transport independently • Work with various insurance types including HMOs, IPAs, Medicare, and MediCAL • Maintain knowledge of Medicare guidelines and medical necessity determinations • Document patient information using Epic charting system • Provide case management services for all patient age groups from neonates to geriatrics • Manage weekend coverage responsibilities overseeing up to 2 units with focus on discharge planning • Complete required orientation hours and annual training modules • Work every other weekend and commit to working 3 of 5 major holidays • Maintain current state licensure and required certifications • Provide minimum of 2 supervisory references covering 1 year of employment in comparable specialty • Utilize facility-provided communication devices when required for assignment duties • Float between facilities within the state as assigned • Complete pre-start and annual training modules with self-reported time tracking • Adhere to facility policies including dress code requirements of business attire or scrubs
Job Requirements:
- Active, unencumbered license in CA
- Minimum of 1-7 years of relevant experience, depending on specialty
- Current Certifications: BLS and/or ACLS, plus any specialty-specific requirements
- Experience with EMR systems (Epic, Cerner, as applicable)
Other Required Documentation:
- Current Resume listing detailed work history (MM/YYYY format)
- Two current clinical references
- State-required professional license
- Physicial exam (completed within the past 12 months)
- TB skin test or Quantiferon Gold (within the past 12 months)
- Immunity Titers - MMR/Hep B/Varicella (declinations may be accepted)
- Current Tdap and Flu vaccinations
- Must pass background check and drug screening
