Position Summary:
VitalHire is seeking a skilled and compassionate LPN - Home Health to join our growing clinician community. This high-impact opportunity offers a 5x8 Days at a leading Rehab in Portland, OR. Key Responsibilities:
• Provide home health nursing services to adult and geriatric patients
• Conduct up to 6 patient visits per day maintaining 30 productivity points standard
• Perform medication assessment and management
• Implement infection control precautions
• Administer ear drops, eye drops, IM & SQ injections, and oral medications
• Manage NG tube and G tube care
• Provide wound care and wound vac management
• Perform subcutaneous injections
• Conduct routine visits and resumption of care
• Complete charting using EPIC Remote Client for Home Health and Rover phone app/Oasis
• Maintain current BLS certification
• Complete 40 hours of orientation
• Complete required modules pre-start and annually (average 16 hours, non-billable)
• Self-report time spent on training modules with completion recorded in Workramp transcripts
• Provide minimum of 2 supervisory references covering 1 year of employment within comparable specialty/setting
• Maintain reliable transportation for patient visits
• Travel within occasional maximum radius of 60 miles (60-90 minute total drive time) per day
• Utilize facility-provided communication devices when required
• Float between facilities within the state as assigned
• Maintain primary series COVID vaccination plus one booster
• Work 8-hour day shifts (facility cannot accommodate 4x10s or 3x12s)
Job Requirements:
- Active, unencumbered license in OR
- Minimum of 1-7 years of relevant experience, depending on specialty
- Current Certifications: BLS and/or ACLS, plus any specialty-specific requirements
- Experience with EMR systems (Epic, Cerner, as applicable)
Other Required Documentation:
- Current Resume listing detailed work history (MM/YYYY format)
- Two current clinical references
- State-required professional license
- Physicial exam (completed within the past 12 months)
- TB skin test or Quantiferon Gold (within the past 12 months)
- Immunity Titers - MMR/Hep B/Varicella (declinations may be accepted)
- Current Tdap and Flu vaccinations
- Must pass background check and drug screening
