Position Summary:
VitalHire is seeking a skilled and compassionate Social Worker - Licensed Master Social Worker to join our growing clinician community. Key Responsibilities:
• Conduct initial evaluations and biopsychosocial assessments for home health patients
• Develop and revise individualized plans of care to meet patient and caregiver needs
• Provide case management and care coordination services
• Document care delivery, plans, goals and patient responses according to coverage criteria
• Document patient and caregiver teaching and learning activities
• Participate in care conference activities with internal and external team members
• Provide and reinforce individualized patient and caregiver education
• Educate patients and caregivers to facilitate safe discharge
• Complete discharge planning and discharge summaries
• Perform up to 6 patient visits per day with 30 productivity points standard
• Use Epic Remote Client for Home Health documentation and Rover phone app
• Complete Oasis documentation requirements
• Provide high risk intervention for patients with vulnerability or safety risks
• Maintain current BLS certification
• Maintain both Oregon and Washington state licensure
• Complete required modules pre-start and annually (average 16 hours, self-reported)
• Provide minimum of 2 supervisory references covering 1 year of employment within comparable specialty
• Provide own transportation for patient visits with occasional 60-mile radius travel
• Float between facilities within the state as required
• Work collaboratively with interdisciplinary team members
• Ensure documentation reflects understanding of Medicare, Medicaid and other coverage criteria
Job Requirements:
- Active, unencumbered license in OR
- Minimum of 1-7 years of relevant experience, depending on specialty
- Current Certifications: BLS and/or ACLS, plus any specialty-specific requirements
- Experience with EMR systems (Epic, Cerner, as applicable)
Other Required Documentation:
- Current Resume listing detailed work history (MM/YYYY format)
- Two current clinical references
- State-required professional license
- Physicial exam (completed within the past 12 months)
- TB skin test or Quantiferon Gold (within the past 12 months)
- Immunity Titers - MMR/Hep B/Varicella (declinations may be accepted)
- Current Tdap and Flu vaccinations
- Must pass background check and drug screening
