Position Summary:
VitalHire is seeking a skilled and compassionate Social Worker - Licensed Master Social Worker to join our growing clinician community. Key Responsibilities:
• Conduct initial patient evaluations and biopsychosocial assessments
• Develop and revise individualized plans of care to meet patient and caregiver needs
• Provide case management and care coordination services
• Document care delivery, plans, goals, and patient responses according to established protocols
• Coordinate with internal and external team members for collaborative care
• Educate patients and caregivers to facilitate discharge and ongoing care
• Participate in care conference activities
• Conduct discharge planning and prepare discharge summaries
• Document patient and caregiver teaching and learning activities
• Provide high-risk intervention for patient vulnerability or safety concerns
• Establish and revise patient care goals
• Document pertinent information to facilitate care coordination
• Ensure documentation reflects understanding of coverage criteria
• Provide and reinforce individualized patient and caregiver education
• Visit up to 6 patients per day maintaining 30 productivity points standard
• Utilize Epic Remote Client for Home Health charting and Rover phone app
• Complete required pre-start and annual training modules
• Maintain current BCLS certification
• Maintain valid Oregon and Washington state licensure
• Provide own transportation for patient visits
• Complete compliance documentation and certifications
• Self-report time spent on non-billable training modules
• Float between facilities within the state as required
Job Requirements:
- Active, unencumbered license in OR
- Minimum of 1-7 years of relevant experience, depending on specialty
- Current Certifications: BLS and/or ACLS, plus any specialty-specific requirements
- Experience with EMR systems (Epic, Cerner, as applicable)
Other Required Documentation:
- Current Resume listing detailed work history (MM/YYYY format)
- Two current clinical references
- State-required professional license
- Physicial exam (completed within the past 12 months)
- TB skin test or Quantiferon Gold (within the past 12 months)
- Immunity Titers - MMR/Hep B/Varicella (declinations may be accepted)
- Current Tdap and Flu vaccinations
- Must pass background check and drug screening
