Position Summary:
VitalHire is seeking a skilled and compassionate Social Worker - Licensed Social Worker to join our growing clinician community. Key Responsibilities:
• Provide psychosocial/emotional support to patients and families in the Emergency Department
• Complete psychosocial assessments and biopsychosocial evaluations
• Identify appropriate discharge plans from a person-centered approach in alignment with policy and regulations
• Provide brief clinical interventions as appropriate
• Assist with resource navigation and coordination of care
• Screen for suicidality and engage in safety planning as appropriate
• Complete appropriate reports of harm based on state statutes
• Coordinate with outside entities and internal/external team members
• Work closely with RN Case Managers and care team to coordinate safe and ethical discharge plans
• Facilitate discharge and contingency planning
• Provide crisis intervention services
• Document care delivery, plans, goals, and patient responses according to plan of care
• Document patient/caregiver teaching and learning
• Educate patients/caregivers to facilitate discharge
• Participate in care conference activities
• Refer to other disciplines and resources as needed
• Utilize Epic charting system for documentation
• Float within the hospital or to affiliate hospitals as needed
• Work 3x12 night shifts with weekend coverage required
• Work 2 out of 3 major holidays (Thanksgiving, Christmas, and New Year)
• Complete required modules and training programs pre-start and annually
• Maintain valid state licensure as LMSW or LCSW
• Provide minimum of 2 supervisory references covering 1 year of employment within comparable specialty/setting
Job Requirements:
- Active, unencumbered license in AK
- Minimum of 1-7 years of relevant experience, depending on specialty
- Current Certifications: BLS and/or ACLS, plus any specialty-specific requirements
- Experience with EMR systems (Epic, Cerner, as applicable)
Other Required Documentation:
- Current Resume listing detailed work history (MM/YYYY format)
- Two current clinical references
- State-required professional license
- Physicial exam (completed within the past 12 months)
- TB skin test or Quantiferon Gold (within the past 12 months)
- Immunity Titers - MMR/Hep B/Varicella (declinations may be accepted)
- Current Tdap and Flu vaccinations
- Must pass background check and drug screening
