Position Summary:
VitalHire is seeking a skilled and compassionate RN - Case Management to join our growing clinician community. This high-impact opportunity offers a 5x8 Days at a leading Hospital in Tarzana, CA. Key Responsibilities:
• Provide case management services for acute care hospital patients with a caseload up to 25 patients
• Utilize Epic charting system for computerized documentation
• Apply InterQual criteria and Milliman Guidelines for medical necessity determinations
• Conduct concurrent reviews and continued stay reviews
• Perform pre-certification reviews and prior authorizations
• Coordinate discharge planning and care coordination activities
• Arrange durable medical equipment (DME), skilled nursing facility (SNF) placements, and ambulance transport independently
• Organize and participate in family meetings
• Participate in the development of treatment plans
• Work with HMOs, IPAs, and other managed care organizations
• Handle appeals and denials processes
• Conduct needs assessments and order DME as required
• Perform retrospective reviews as needed
• Ensure compliance with HIPAA guidelines and regulatory requirements
• Work weekend rotation shifts as assigned
• Maintain case management for adult and geriatric patient populations
• Complete required modules and training programs pre-start and annually
• Work 3 of the 5 major holidays (Thanksgiving, Christmas Eve, Christmas Day, New Year's Eve, New Year's Day)
• Float between facilities within the state as required
• Provide minimum of 2 supervisory references covering 1 year of employment within comparable specialty/setting
• Self-report time spent on non-billable training modules
• Utilize facility-provided communication devices when required for assignment duties
Job Requirements:
- Active, unencumbered license in CA
- Minimum of 1-7 years of relevant experience, depending on specialty
- Current Certifications: BLS and/or ACLS, plus any specialty-specific requirements
- Experience with EMR systems (Epic, Cerner, as applicable)
Other Required Documentation:
- Current Resume listing detailed work history (MM/YYYY format)
- Two current clinical references
- State-required professional license
- Physicial exam (completed within the past 12 months)
- TB skin test or Quantiferon Gold (within the past 12 months)
- Immunity Titers - MMR/Hep B/Varicella (declinations may be accepted)
- Current Tdap and Flu vaccinations
- Must pass background check and drug screening
