Position Summary:
VitalHire is seeking a skilled and compassionate RN - Case Management to join our growing clinician community. Key Responsibilities:
• Perform case management services for acute care hospital patients including adolescents, adults, and geriatrics
• Conduct care coordination and discharge planning activities
• Assess patients using InterQual admission criteria
• Develop and participate in treatment plans
• Organize and participate in family meetings
• Arrange durable medical equipment (DME), skilled nursing facility (SNF) placements, and ambulance transport independently
• Work with patients experiencing homelessness, mental health crises, and substance abuse issues
• Coordinate care for patients requiring advance directives, POLST, and protective services
• Work with HMOs, IPAs, and other insurance organizations
• Maintain patient ratios of approximately 20 patients during day shifts
• Utilize Epic charting system for computerized documentation
• Work every other weekend as required
• Commit to working 3 of the 5 major holidays (Thanksgiving, Christmas Eve, Christmas Day, New Year's Eve, New Year's Day)
• Float between facilities within the state as assigned
• Complete required modules pre-start and annually
• Collaborate with interdisciplinary team including social services and transportation
• Use facility-provided communication devices when required
• Maintain proficiency in Microsoft applications
• Complete required skills checklist and compliance documentation
• Provide minimum of 2 supervisory references covering 1 year of employment within comparable specialty/setting
• Self-report time spent on non-billable training modules
Job Requirements:
- Active, unencumbered license in CA
- Minimum of 1-7 years of relevant experience, depending on specialty
- Current Certifications: BLS and/or ACLS, plus any specialty-specific requirements
- Experience with EMR systems (Epic, Cerner, as applicable)
Other Required Documentation:
- Current Resume listing detailed work history (MM/YYYY format)
- Two current clinical references
- State-required professional license
- Physicial exam (completed within the past 12 months)
- TB skin test or Quantiferon Gold (within the past 12 months)
- Immunity Titers - MMR/Hep B/Varicella (declinations may be accepted)
- Current Tdap and Flu vaccinations
- Must pass background check and drug screening
