Position Summary:
VitalHire is seeking a skilled and compassionate RN - Case Management to join our growing clinician community. Key Responsibilities:
• Provide case management services for RN role with focus on care coordination and discharge planning
• Maintain patient caseload of approximately 20 patients during day shifts
• Utilize Epic charting system for computerized documentation
• Apply Utilization Review knowledge and InterQual criteria for admission assessments
• Assess patients and coordinate care across the continuum
• Develop and participate in treatment plan creation
• Organize and participate in family meetings
• Coordinate with interdisciplinary support including Social Services and Transportation
• Arrange DME, SNF placement, and ambulance transport independently
• Work with patients experiencing drug/alcohol abuse, homelessness, mental health crises
• Handle advance directives, POLST, and protective services processes
• Work with HMOs, IPAs, and other insurance providers
• Provide care for adolescents, adults, and geriatric patients
• Work every other weekend as required
• Commit to working 3 of 5 major holidays (Thanksgiving, Christmas Eve, Christmas Day, New Year's Eve, New Year's Day)
• Float between facilities within the state as needed
• Complete 40 hours of orientation training
• Complete required pre-start and annual training modules
• Maintain proficiency in Microsoft applications
• Provide minimum of 2 supervisory references covering 1 year of employment in comparable specialty
• Self-report time spent on non-billable training modules
Job Requirements:
- Active, unencumbered license in CA
- Minimum of 1-7 years of relevant experience, depending on specialty
- Current Certifications: BLS and/or ACLS, plus any specialty-specific requirements
- Experience with EMR systems (Epic, Cerner, as applicable)
Other Required Documentation:
- Current Resume listing detailed work history (MM/YYYY format)
- Two current clinical references
- State-required professional license
- Physicial exam (completed within the past 12 months)
- TB skin test or Quantiferon Gold (within the past 12 months)
- Immunity Titers - MMR/Hep B/Varicella (declinations may be accepted)
- Current Tdap and Flu vaccinations
- Must pass background check and drug screening
