Position Summary:
VitalHire is seeking a skilled and compassionate Social Worker - Licensed Social Worker to join our growing clinician community. Key Responsibilities:
• Provide hospital care management and discharge planning services as a licensed social worker
• Conduct initial patient evaluations and assessments
• Develop and revise care plans to meet patient and caregiver needs
• Coordinate care with internal and external team members
• Facilitate discharge and contingency planning for patients
• Arrange transfers to physician appointments and coordinate discharge home
• Coordinate services to home care and equipment following discharge
• Refer patients to rehabilitation and long-term care facilities as needed
• Provide and reinforce individualized patient and caregiver education
• Document patient care according to plan of care and coverage criteria
• Document patient response to treatment and pertinent care coordination information
• Participate in care conference activities
• Complete all required paperwork and accountability reports
• Utilize Epic charting system for all documentation
• Work with diverse patient populations including adults with various medical conditions
• Handle cases involving advance directives, guardianship, and power of attorney
• Coordinate with Medicare, Medicaid, and other benefit programs
• Address high-risk patient vulnerability and safety concerns
• Work 2 weekend shifts every pay period
• Float between hospital units and affiliate facilities as needed
• Complete required pre-start and annual training modules
• Maintain current Oregon licensure and required certifications
• Provide 2 supervisory references covering 1 year of experience
• Work 3 out of 5 major holidays (Thanksgiving, Christmas Eve, Christmas Day, New Year's Eve, New Year's Day)
• Use facility-provided communication devices when required
• Self-report time spent on non-billable training modules
Job Requirements:
- Active, unencumbered license in OR
- Minimum of 1-7 years of relevant experience, depending on specialty
- Current Certifications: BLS and/or ACLS, plus any specialty-specific requirements
- Experience with EMR systems (Epic, Cerner, as applicable)
Other Required Documentation:
- Current Resume listing detailed work history (MM/YYYY format)
- Two current clinical references
- State-required professional license
- Physicial exam (completed within the past 12 months)
- TB skin test or Quantiferon Gold (within the past 12 months)
- Immunity Titers - MMR/Hep B/Varicella (declinations may be accepted)
- Current Tdap and Flu vaccinations
- Must pass background check and drug screening
