Position Summary:
VitalHire is seeking a skilled and compassionate PT - Physical Therapist to join our growing clinician community. This high-impact opportunity offers a 5x8 Days at a leading SNF in Portland, OR. Key Responsibilities:
• Provide physical therapy services in home health settings
• Conduct patient evaluations and assessments
• Visit up to 6 patients per day meeting 30 productivity points standard
• Complete OASIS documentation requirements
• Utilize Epic Remote Client for Home Health charting on laptop and Rover phone app
• Develop and implement home exercise programs for patients
• Provide patient and family education
• Perform gait training and mobility assessments
• Conduct strength testing and range of motion exercises
• Assess and recommend assistive devices and adaptive equipment
• Manage pain assessment and treatment interventions
• Create and monitor short-term and long-term goals
• Complete reassessments and discharge planning
• Maintain current BLS certification
• Provide treatment for various diagnoses including dementia, Parkinson's, ALS, MS, orthopedic post-op, and post-stroke patients
• Perform lifting and transfer techniques safely
• Schedule patient appointments and order supplies
• Complete required modules pre-start and annually
• Provide minimum of 2 supervisory references covering 1 year of employment within comparable specialty
• Maintain reliable transportation for patient visits
• Travel within approximately 60-mile radius with 60-90 minute total drive time per day
• Self-report time spent on non-billable training modules
• Float between facilities within the state as required
Job Requirements:
- Active, unencumbered license in OR
- Minimum of 1-7 years of relevant experience, depending on specialty
- Current Certifications: BLS and/or ACLS, plus any specialty-specific requirements
- Experience with EMR systems (Epic, Cerner, as applicable)
Other Required Documentation:
- Current Resume listing detailed work history (MM/YYYY format)
- Two current clinical references
- State-required professional license
- Physicial exam (completed within the past 12 months)
- TB skin test or Quantiferon Gold (within the past 12 months)
- Immunity Titers - MMR/Hep B/Varicella (declinations may be accepted)
- Current Tdap and Flu vaccinations
- Must pass background check and drug screening
