Position Summary:

Key Responsibilities:
• Provide physical therapy services in home health setting with 1 year of skilled home health experience
• Conduct patient visits with up to 6 patients per day maintaining 30 productivity points standard
• Perform comprehensive evaluations, assessments, and reassessments of patients
• Complete OASIS documentation requirements for home health patients
• Utilize EPIC Remote Client for Home Health charting on laptop and Rover phone app
• Develop and implement home exercise programs for patients
• Provide gait training and assistive device instruction
• Conduct strength testing and range of motion exercises (AROM/AAROM/PROM)
• Perform patient and family education on treatment plans and safety measures
• Maintain current BCLS certification
• Provide reliable transportation for patient visits with occasional travel up to 60 miles radius
• Complete pre-start and annual training modules averaging 16 hours
• Maintain professional business casual attire or scrubs as appropriate
• Document treatment plans, progress notes, and discharge planning
• Coordinate care with interdisciplinary team members
• Manage scheduling appointments and ordering supplies
• Perform lifting and transfer techniques safely
• Complete required compliance documentation and certifications
• Provide minimum of 2 supervisory references covering 1 year of employment within comparable specialty
• Self-report time spent on non-billable training modules
• Utilize facility-provided communication devices when required
Job Requirements:
- Active, unencumbered license in OR
- Minimum of 1-7 years of relevant experience, depending on specialty
- Current Certifications: BLS and/or ACLS, plus any specialty-specific requirements
- Experience with EMR systems (Epic, Cerner, as applicable)
Other Required Documentation:
- Current Resume listing detailed work history (MM/YYYY format)
- Two current clinical references
- State-required professional license
- Physicial exam (completed within the past 12 months)
- TB skin test or Quantiferon Gold (within the past 12 months)
- Immunity Titers - MMR/Hep B/Varicella (declinations may be accepted)
- Current Tdap and Flu vaccinations
- Must pass background check and drug screening
