Position Summary:
VitalHire is seeking a skilled and compassionate PT - Physical Therapist to join our growing clinician community. Key Responsibilities:
• Provide physical therapy services to adult patients in home health settings • Visit 4-6 patients per day with 27-30 productivity points standard based on territory • Complete OASIS assessments and Start of Care (SOC) documentation • Maintain current BLS certification and valid state licensure • Document patient care using Home Care Home Base (HCHB) charting system • Provide timely communication and documentation as required • Travel to patient homes within assigned territory with occasional radius up to 100 miles • Maintain own transportation and valid driver's license • Complete required compliance documentation and training • Participate in on-call rotation as assigned • Treat various conditions including post-operative care, spinal conditions, joint replacements, neurological disorders, and traumatic injuries • Develop and implement home exercise programs for patients • Provide patient and family education • Return all provided documentation tools and equipment as required • Adhere to facility policies and procedures for patient care delivery
Job Requirements:
- Active, unencumbered license in WA
- Minimum of 1-7 years of relevant experience, depending on specialty
- Current Certifications: BLS and/or ACLS, plus any specialty-specific requirements
- Experience with EMR systems (Epic, Cerner, as applicable)
Other Required Documentation:
- Current Resume listing detailed work history (MM/YYYY format)
- Two current clinical references
- State-required professional license
- Physicial exam (completed within the past 12 months)
- TB skin test or Quantiferon Gold (within the past 12 months)
- Immunity Titers - MMR/Hep B/Varicella (declinations may be accepted)
- Current Tdap and Flu vaccinations
- Must pass background check and drug screening
