Position Summary:
VitalHire is seeking a skilled and compassionate PT - Physical Therapist to join our growing clinician community. Key Responsibilities:
• Provide physical therapy services in home health settings for geriatric patients
• Complete 6 daily visits or minimum of 30 points weekly for productivity
• Manage average caseload of 5-7 patients per day
• Conduct Start of Care (SOC) assessments using OASIS experience
• Utilize HCHB (Homecare Homebase) charting system for documentation
• Maintain timely documentation and communication requirements
• Travel within 60-mile radius territory from office hub
• Provide flexible coverage for extended territory based on patient needs
• Maintain current BLS certification and valid state licensure
• Use facility-provided iPad for patient care documentation
• Complete on-call rotation requirements
• Provide patient and family education on home exercise programs
• Treat various conditions including post-operative cases, spinal conditions, joint replacements, and neurological disorders
• Complete mandatory Hospice/Home Health Checklist
• Submit Requested Time Off (RTO) at time of application
• Maintain active driver's license and personal transportation
• Adhere to facility dress code requirements
Job Requirements:
- Active, unencumbered license in WA
- Minimum of 1-7 years of relevant experience, depending on specialty
- Current Certifications: BLS and/or ACLS, plus any specialty-specific requirements
- Experience with EMR systems (Epic, Cerner, as applicable)
Other Required Documentation:
- Current Resume listing detailed work history (MM/YYYY format)
- Two current clinical references
- State-required professional license
- Physicial exam (completed within the past 12 months)
- TB skin test or Quantiferon Gold (within the past 12 months)
- Immunity Titers - MMR/Hep B/Varicella (declinations may be accepted)
- Current Tdap and Flu vaccinations
- Must pass background check and drug screening
