Position Summary:
VitalHire is seeking a skilled and compassionate PT - Physical Therapist to join our growing clinician community. Key Responsibilities:
• Provide physical therapy services to patients in home health setting
• Complete OASIS assessments for Start of Care (SOC) visits
• Conduct 6 visits daily with minimum of 30 points weekly for productivity
• Manage caseload of 5-7 patients per day on average
• Provide care for geriatric patients with various diagnoses
• Document patient care using HCHB (Homecare Homebase) system
• Maintain current BLS certification and state licensure
• Participate in on-call rotation every 3rd to 4th weekend
• Travel within 60-mile radius territory from hub office
• Provide timely documentation and communication
• Complete patient and family education on home exercise programs
• Treat conditions including post-operative care, spinal conditions, joint replacements, and neurological disorders
• Maintain professional appearance in scrubs (no jeans)
• Use facility-provided iPad for documentation and communication
• Submit requested time off at time of application
• Complete required Hospice/Home Health Checklist
• Maintain active driver's license and personal transportation
• Demonstrate flexibility with territory assignments
Job Requirements:
- Active, unencumbered license in WA
- Minimum of 1-7 years of relevant experience, depending on specialty
- Current Certifications: BLS and/or ACLS, plus any specialty-specific requirements
- Experience with EMR systems (Epic, Cerner, as applicable)
Other Required Documentation:
- Current Resume listing detailed work history (MM/YYYY format)
- Two current clinical references
- State-required professional license
- Physicial exam (completed within the past 12 months)
- TB skin test or Quantiferon Gold (within the past 12 months)
- Immunity Titers - MMR/Hep B/Varicella (declinations may be accepted)
- Current Tdap and Flu vaccinations
- Must pass background check and drug screening
