Position Summary:
VitalHire is seeking a skilled and compassionate Social Worker - Licensed Master Social Worker to join our growing clinician community. Key Responsibilities:
• Provide social work services to adults 55 and older who are low income, medically complex, with mental health diagnoses, behaviors, and/or substance abuse issues
• Conduct biopsychosocial assessments and initial evaluations
• Develop and revise individualized plans of care to meet patient and caregiver needs
• Provide individual patient care and facilitate group counseling sessions
• Coordinate quality care in a cost-effective manner with internal and external team members
• Participate in care conference activities and collaborative care planning
• Conduct discharge planning and facilitate discharge and contingency planning
• Provide crisis intervention services when needed
• Document patient care, treatment plans, goals, and patient responses according to established protocols
• Educate patients and caregivers to facilitate discharge and ongoing care
• Refer patients to other disciplines and community resources as appropriate
• Assess patient response to treatment and modify care plans accordingly
• Complete Mini-Mental State Examinations (MMSE) and Geriatric Depression Scales
• Address cultural issues and provide culturally competent care
• Handle cases involving suicide, homicide, and self-harm risk assessments
• Utilize EPIC EMR system for all documentation and charting
• Maintain current BLS certification
• Complete required pre-start and annual training modules
• Provide minimum of 2 supervisory references covering 1 year of employment within comparable specialty/setting
• Float between facilities within the state as required
• Self-report time spent on non-billable training modules
• Use facility-provided communication devices when required for assignment duties
Job Requirements:
- Active, unencumbered license in WA
- Minimum of 1-7 years of relevant experience, depending on specialty
- Current Certifications: BLS and/or ACLS, plus any specialty-specific requirements
- Experience with EMR systems (Epic, Cerner, as applicable)
Other Required Documentation:
- Current Resume listing detailed work history (MM/YYYY format)
- Two current clinical references
- State-required professional license
- Physicial exam (completed within the past 12 months)
- TB skin test or Quantiferon Gold (within the past 12 months)
- Immunity Titers - MMR/Hep B/Varicella (declinations may be accepted)
- Current Tdap and Flu vaccinations
- Must pass background check and drug screening
