Position Summary:
VitalHire is seeking a skilled and compassionate OT - Therapy Rehab to join our growing clinician community. Key Responsibilities:
• Provide occupational therapy services in home health setting with 1 year of recent skilled hospice experience
• Visit up to 6 patients per day maintaining 27-30 productivity points standard
• Complete OASIS documentation for Start of Care (SOC)
• Maintain current BLS certification and Washington state license with NBCOT
• Utilize HomeCare HomeBase charting system for documentation
• Provide timely communication and documentation as required
• Travel up to 100 mile radius using personal transportation
• Participate in on-call rotation as assigned
• Complete required compliance documentation within 3 weeks
• Submit Requested Time Off (RTO) at time of application
• Complete mandatory Hospice/Home Health Checklist
• Maintain active driver's license and personal transportation
• Return all provided documentation tools upon completion
• Work with adult patient population providing occupational therapy services
• Demonstrate proficiency in various OT skills including ADLs, cardiac rehabilitation, prosthetics, and splinting techniques
Job Requirements:
- Active, unencumbered license in WA
- Minimum of 1-7 years of relevant experience, depending on specialty
- Current Certifications: BLS and/or ACLS, plus any specialty-specific requirements
- Experience with EMR systems (Epic, Cerner, as applicable)
Other Required Documentation:
- Current Resume listing detailed work history (MM/YYYY format)
- Two current clinical references
- State-required professional license
- Physicial exam (completed within the past 12 months)
- TB skin test or Quantiferon Gold (within the past 12 months)
- Immunity Titers - MMR/Hep B/Varicella (declinations may be accepted)
- Current Tdap and Flu vaccinations
- Must pass background check and drug screening
