Position Summary:
VitalHire is seeking a skilled and compassionate PT - Physical Therapist to join our growing clinician community. This high-impact opportunity offers a 5x8 Days at a leading LTC in Spokane Valley, WA. Key Responsibilities:
• Provide occupational therapy services in home health setting
• Complete 6 visits daily and maintain minimum of 30 points weekly for productivity
• Manage average caseload of 5-7 patients per day
• Work with geriatric patient population
• Utilize OASIS documentation for Start of Care (SOC)
• Use HCHB (Homecare Homebase) electronic medical record system
• Maintain timely documentation and communication
• Participate in on-call rotation every 3rd to 4th weekend
• Travel within 60-mile radius territory from office hub
• Maintain flexibility with assigned territory coverage
• Maintain current BLS certification
• Maintain valid state licensure
• Complete required Hospice/Home Health Checklist
• Maintain active driver's license and personal transportation
• Adhere to dress code requirements (scrubs, no jeans)
• Utilize facility-provided iPad for documentation
• Provide occupational therapy services for various conditions including post-operative care, neurological conditions, and rehabilitation needs
Job Requirements:
- Active, unencumbered license in WA
- Minimum of 1-7 years of relevant experience, depending on specialty
- Current Certifications: BLS and/or ACLS, plus any specialty-specific requirements
- Experience with EMR systems (Epic, Cerner, as applicable)
Other Required Documentation:
- Current Resume listing detailed work history (MM/YYYY format)
- Two current clinical references
- State-required professional license
- Physicial exam (completed within the past 12 months)
- TB skin test or Quantiferon Gold (within the past 12 months)
- Immunity Titers - MMR/Hep B/Varicella (declinations may be accepted)
- Current Tdap and Flu vaccinations
- Must pass background check and drug screening
