Position Summary:
VitalHire is seeking a skilled and compassionate OT - Therapy Rehab to join our growing clinician community. This high-impact opportunity offers a 5x8 Days at a leading Hospital in Santa Rosa, CA. Key Responsibilities:
• Provide occupational therapy services in acute rehabilitation setting
• Conduct assessments for acute and chronic disorders
• Perform functional activities assessments and interventions
• Implement infection prevention protocols
• Conduct fall and risk assessments
• Provide patient care for adults and geriatric populations
• Utilize Epic charting system for documentation
• Float between acute rehab units and affiliate hospitals as needed
• Maintain current BLS certification
• Apply manual therapy and hand therapy techniques
• Treat patients with neurological diagnoses including traumatic brain injury, spinal cord injury, CVA, and Parkinson's disease
• Provide rehabilitation services for amputees, arthritis, and orthopedic conditions
• Conduct standardized assessments including Functional Independence Measure (FIM)
• Provide adaptive equipment assessments for adults
• Implement discharge planning and patient/family education
• Complete pre-start and annual training modules
• Work 2 out of 3 major holidays (Thanksgiving, Christmas, and New Year)
• Collaborate with interdisciplinary team including physical therapy, respiratory services, and other support services
• Maintain valid state licensure and required certifications
• Provide minimum of 2 supervisory references covering 1 year of employment within comparable specialty/setting
• Self-report time spent on non-billable training modules
• Utilize facility-provided communication devices when required
• Adhere to National Patient Safety Goals and Core Measures
• Follow facility dress code requirements
Job Requirements:
- Active, unencumbered license in CA
- Minimum of 1-7 years of relevant experience, depending on specialty
- Current Certifications: BLS and/or ACLS, plus any specialty-specific requirements
- Experience with EMR systems (Epic, Cerner, as applicable)
Other Required Documentation:
- Current Resume listing detailed work history (MM/YYYY format)
- Two current clinical references
- State-required professional license
- Physicial exam (completed within the past 12 months)
- TB skin test or Quantiferon Gold (within the past 12 months)
- Immunity Titers - MMR/Hep B/Varicella (declinations may be accepted)
- Current Tdap and Flu vaccinations
- Must pass background check and drug screening
