Position Summary:
VitalHire is seeking a skilled and compassionate PT - Physical Therapist to join our growing clinician community. This high-impact opportunity offers a 5x8 Days at a leading LTC in Mount Angel, OR. Key Responsibilities:
• Provide physical therapy services in home health setting
• Conduct patient visits with up to 6 patients per day maintaining 30 productivity points standard
• Complete OASIS documentation as required
• Utilize EPIC Remote Client for Home Health charting on laptop and Rover phone app
• Maintain current BLS certification
• Provide care to adult and geriatric patient populations
• Travel to patient homes within 60-mile radius with occasional 60-90 minute total drive time per day
• Maintain reliable transportation for patient visits
• Complete 16 hours of orientation upon start
• Complete required pre-start and annual training modules
• Self-report time spent on non-billable training modules
• Maintain valid state physical therapy licensure
• Provide minimum of 2 supervisory references covering 1 year of employment within comparable specialty setting
• Work 8-hour day shifts Monday through Friday
• Adhere to business casual dress code or scrubs
• Float between facilities within the state as required
• Utilize facility-provided laptop and work cell phone for documentation and communication
• Complete primary series COVID vaccination plus one booster
• Maintain skills in geriatrics, orthopedics, and OASIS documentation
Job Requirements:
- Active, unencumbered license in OR
- Minimum of 1-7 years of relevant experience, depending on specialty
- Current Certifications: BLS and/or ACLS, plus any specialty-specific requirements
- Experience with EMR systems (Epic, Cerner, as applicable)
Other Required Documentation:
- Current Resume listing detailed work history (MM/YYYY format)
- Two current clinical references
- State-required professional license
- Physicial exam (completed within the past 12 months)
- TB skin test or Quantiferon Gold (within the past 12 months)
- Immunity Titers - MMR/Hep B/Varicella (declinations may be accepted)
- Current Tdap and Flu vaccinations
- Must pass background check and drug screening
