Position Summary:
VitalHire is seeking a skilled and compassionate Pharmacist - Pharmacist to join our growing clinician community. Key Responsibilities:
• Provide comprehensive pharmaceutical services in an acute care hospital setting • Work variable start times including 5am, 6am, 10:30am, or 3:30pm shifts • Utilize Epic charting system for all documentation and pharmaceutical records • Manage and monitor high-alert medications, anticoagulants, and chemotherapy protocols • Perform clinical dosing for cardiac, geriatric, pediatric, renal, and pharmacokinetic applications • Conduct therapeutic medication monitoring and warfarin dosing • Operate and maintain Pyxis medication dispensing systems • Prepare and compound IV piggyback and IV push medications • Handle packaging of antineoplastic agents following safety protocols • Float between sister facilities and affiliate hospitals as needed • Provide pharmaceutical services across all patient age groups including neonates, infants, pediatrics, adolescents, adults, and geriatrics • Support Level II Trauma Center operations and emergency pharmaceutical needs • Work weekends based on department needs and staffing requirements • Work 2 out of 3 major holidays (Thanksgiving, Christmas, and New Year) • Complete required pre-start and annual training modules • Maintain current state licensure and required certifications including chemo/hazardous drug and IV certification • Provide minimum of 2 supervisory references covering 1 year of employment within comparable specialty/setting • Comply with facility policies and procedures for pharmaceutical care delivery • Self-report time spent on non-billable training modules • Utilize facility-provided communication devices when required for assignment duties
Job Requirements:
- Active, unencumbered license in CA
- Minimum of 1-7 years of relevant experience, depending on specialty
- Current Certifications: BLS and/or ACLS, plus any specialty-specific requirements
- Experience with EMR systems (Epic, Cerner, as applicable)
Other Required Documentation:
- Current Resume listing detailed work history (MM/YYYY format)
- Two current clinical references
- State-required professional license
- Physicial exam (completed within the past 12 months)
- TB skin test or Quantiferon Gold (within the past 12 months)
- Immunity Titers - MMR/Hep B/Varicella (declinations may be accepted)
- Current Tdap and Flu vaccinations
- Must pass background check and drug screening
