Position Summary:
VitalHire is seeking a skilled and compassionate RN - Case Management to join our growing clinician community. This high-impact opportunity offers a 5x8 Days at a leading Hospital in Burbank, CA. Key Responsibilities:
• Perform case management duties for adult and geriatric patients with caseloads up to 25 patients • Conduct care coordination and discharge planning activities • Develop comprehensive plans of care to meet patient and caregiver needs • Perform needs assessments and order durable medical equipment (DME) • Participate in care conference activities and family meetings • Collaborate with interdisciplinary team members in treatment plan development • Provide social work services for patients with complex psychosocial needs • Address issues related to drug/alcohol abuse treatment options, homelessness, and mental health crises • Facilitate advance directive and POLST discussions • Navigate protective services processes when indicated • Utilize Epic charting system for documentation and care coordination • Complete required pre-start and annual training modules • Work typically one full weekend per 4 weeks as needed by department • Commit to working 3 of the 5 major holidays (Thanksgiving, Christmas Eve, Christmas Day, New Year's Eve, New Year's Day) • Float between facilities within the state as assigned • Maintain current state licensure and required certifications • Provide minimum of 2 supervisory references covering 1 year of employment within comparable specialty/setting • Complete skills checklist and compliance documentation • Utilize facility-provided communication devices when required for assignment duties • Self-report time spent on non-billable training modules • Complete 40-hour orientation program
Job Requirements:
- Active, unencumbered license in CA
- Minimum of 1-7 years of relevant experience, depending on specialty
- Current Certifications: BLS and/or ACLS, plus any specialty-specific requirements
- Experience with EMR systems (Epic, Cerner, as applicable)
Other Required Documentation:
- Current Resume listing detailed work history (MM/YYYY format)
- Two current clinical references
- State-required professional license
- Physicial exam (completed within the past 12 months)
- TB skin test or Quantiferon Gold (within the past 12 months)
- Immunity Titers - MMR/Hep B/Varicella (declinations may be accepted)
- Current Tdap and Flu vaccinations
- Must pass background check and drug screening
