Position Summary:
VitalHire is seeking a skilled and compassionate RN - Home Health to join our growing clinician community. This high-impact opportunity offers a 5x8 Days at a leading LTC in Medford, OR. Key Responsibilities:
• Provide skilled home health nursing services to patients in their homes
• Conduct admission assessments for new patients
• Perform start of care visits and documentation
• Complete routine visit assessments and care delivery
• Manage patient recertification processes
• Handle discharge planning and discharge due to safety or non-compliance situations
• Provide resumption of care services when appropriate
• Administer medications and manage medication assessments
• Perform wound care and wound management
• Conduct pain assessment and management using various pain scales
• Manage PCA pump operations when required
• Administer injections via intradermal, intramuscular, and subcutaneous routes
• Manage intravenous infusions as needed
• Implement infection control precautions during patient care
• Complete OASIS documentation and assessments
• Utilize computerized documentation systems including EPIC
• Maintain current BLS certification
• Provide case management services to assigned patients
• Visit up to 6 patients per day meeting 30 productivity points standard
• Complete required pre-start and annual training modules
• Float between facilities within the state as required
• Provide minimum of 2 supervisory references covering 1 year of employment
• Maintain reliable transportation for patient visits
• Self-report time spent on non-billable training modules
• Utilize facility-provided communication devices when required
• Complete required compliance documentation and certifications
Job Requirements:
- Active, unencumbered license in OR
- Minimum of 1-7 years of relevant experience, depending on specialty
- Current Certifications: BLS and/or ACLS, plus any specialty-specific requirements
- Experience with EMR systems (Epic, Cerner, as applicable)
Other Required Documentation:
- Current Resume listing detailed work history (MM/YYYY format)
- Two current clinical references
- State-required professional license
- Physicial exam (completed within the past 12 months)
- TB skin test or Quantiferon Gold (within the past 12 months)
- Immunity Titers - MMR/Hep B/Varicella (declinations may be accepted)
- Current Tdap and Flu vaccinations
- Must pass background check and drug screening
