Position Summary:
VitalHire is seeking a skilled and compassionate RN - Case Management to join our growing clinician community. This high-impact opportunity offers a 4x10 Days at a leading Hospital in Medford, OR. Key Responsibilities:
• Provide case management services as a Registered Nurse for inpatient adult units including medical/telemetry, ICU, general medical, observation unit, and emergency department
• Assist patients and families to develop and fulfill safe discharge plans with available resources
• Coordinate durable medical equipment, home health, hospice, shelters, and post-acute placements such as Skilled Nursing Facilities, adult foster homes, and assisted living
• Work with multiple community partners to arrange discharge plans and secure authorization from insurance companies
• Float between all acute departments in the hospital including ICU, medical, med-surg, telemetry, observation unit, and emergency department
• Maintain Epic charting system proficiency for documentation
• Maintain current BLS certification
• Work weekend shifts as required with 4x10 hour schedule
• Manage patient caseloads with ratios of 1:16 days, 1:18 days, and 1:32 weekends
• Collaborate with Case Management Assistants on tasks including SNF authorization, transport coordination, DME dispensing, and placement referrals
• Focus weekend coverage on discharges and new assessments while dividing responsibilities by units with team assistance
• Complete 40 hours of orientation training
• Complete required pre-start and annual training modules
• Provide care for adult and geriatric patient populations
• Work with interdisciplinary support teams including IV Teams, Physical Therapy, Respiratory Services, Social Services, and other hospital departments
• Maintain valid state RN licensure
• Provide 2 supervisory references covering 1 year of experience within the last 3 years
• Commit to working 3 of the 5 major holidays (Thanksgiving, Christmas Eve, Christmas Day, New Year's Eve, New Year's Day)
• Float to affiliate hospitals as needed
• Maintain reliable transportation for assignment requirements
• Self-report time spent on non-billable training modules
Job Requirements:
- Active, unencumbered license in OR
- Minimum of 1-7 years of relevant experience, depending on specialty
- Current Certifications: BLS and/or ACLS, plus any specialty-specific requirements
- Experience with EMR systems (Epic, Cerner, as applicable)
Other Required Documentation:
- Current Resume listing detailed work history (MM/YYYY format)
- Two current clinical references
- State-required professional license
- Physicial exam (completed within the past 12 months)
- TB skin test or Quantiferon Gold (within the past 12 months)
- Immunity Titers - MMR/Hep B/Varicella (declinations may be accepted)
- Current Tdap and Flu vaccinations
- Must pass background check and drug screening
