Position Summary:
VitalHire is seeking a skilled and compassionate RN - Home Health to join our growing clinician community. This high-impact opportunity offers a 5x8 Days at a leading Rehab in Medford, OR. Key Responsibilities:
• Provide home health nursing services to adult patients in their homes
• Conduct admission assessments and start of care visits
• Perform routine visits with up to 6 patients per day (30 productivity points standard)
• Complete discharge planning and discharge visits due to safety or non-compliance
• Manage recertification processes and resumption of care visits
• Administer medications and provide medication assessment and management
• Provide wound care and utilize appropriate infection control precautions
• Perform pain assessment and management using verbal and nonverbal pain scales
• Manage PCA pump operations when required
• Administer medications via intradermal, intramuscular, subcutaneous, and intravenous infusion routes
• Complete OASIS documentation and maintain computerized documentation in EPIC system
• Provide case management services as needed
• Maintain current BLS certification
• Utilize reliable transportation for patient visits with occasional travel up to 50+ mile radius
• Complete required pre-start and annual training modules (average 16 hours)
• Float between facilities within the state as required
• Provide minimum of 2 supervisory references covering 1 year of employment within comparable specialty/setting
• Self-report time spent on non-billable training modules
• Maintain primary series COVID vaccination plus one booster
• Use facility-provided communication devices when required for patient care duties
Job Requirements:
- Active, unencumbered license in OR
- Minimum of 1-7 years of relevant experience, depending on specialty
- Current Certifications: BLS and/or ACLS, plus any specialty-specific requirements
- Experience with EMR systems (Epic, Cerner, as applicable)
Other Required Documentation:
- Current Resume listing detailed work history (MM/YYYY format)
- Two current clinical references
- State-required professional license
- Physicial exam (completed within the past 12 months)
- TB skin test or Quantiferon Gold (within the past 12 months)
- Immunity Titers - MMR/Hep B/Varicella (declinations may be accepted)
- Current Tdap and Flu vaccinations
- Must pass background check and drug screening
