Position Summary:
VitalHire is seeking a skilled and compassionate RN - Home Health to join our growing clinician community. This high-impact opportunity offers a 5x8 Days at a leading SNF in Medford, OR. Key Responsibilities:
• Provide skilled home health nursing services to adult patients
• Conduct admission assessments for new patients
• Perform start of care visits and documentation
• Complete routine home health visits with up to 6 patients per day
• Meet 30 productivity points standard per day
• Perform recertification assessments and documentation
• Handle discharge planning and discharge due to safety or non-compliance situations
• Manage resumption of care for returning patients
• Provide wound care assessment and treatment
• Administer and manage medications including intradermal, intramuscular, subcutaneous, and intravenous infusions
• Manage PCA pump therapy
• Conduct pain assessment and management using various pain scales
• Implement infection control precautions
• Complete OASIS documentation requirements
• Utilize EPIC charting system for computerized documentation
• Maintain current BLS certification
• Complete required pre-start and annual training modules
• Self-report time spent on non-billable training modules
• Float between facilities within the state as required
• Provide minimum of 2 supervisory references covering 1 year of employment
• Maintain reliable transportation for patient visits
• Utilize facility-provided communication devices when required
• Complete required COVID vaccination series including booster
Job Requirements:
- Active, unencumbered license in OR
- Minimum of 1-7 years of relevant experience, depending on specialty
- Current Certifications: BLS and/or ACLS, plus any specialty-specific requirements
- Experience with EMR systems (Epic, Cerner, as applicable)
Other Required Documentation:
- Current Resume listing detailed work history (MM/YYYY format)
- Two current clinical references
- State-required professional license
- Physicial exam (completed within the past 12 months)
- TB skin test or Quantiferon Gold (within the past 12 months)
- Immunity Titers - MMR/Hep B/Varicella (declinations may be accepted)
- Current Tdap and Flu vaccinations
- Must pass background check and drug screening
