Position Summary:
VitalHire is seeking a skilled and compassionate OT - Therapy Rehab to join our growing clinician community. This high-impact opportunity offers a 5x8 Days at a leading Hospital in Flagstaff, AZ. Key Responsibilities:
• Provide 1:1 direct occupational therapy care to patients
• Manage caseload of 8-11 patients per 8-hour work day
• Deliver outpatient occupational therapy services with emphasis on hand therapy
• Work in multidisciplinary clinic environment with OT, PT, and SLP services
• Collaborate with in-network orthopedics, neuro, and primary care providers
• Maintain current AZ occupational therapy license
• Maintain current BLS certification
• Obtain and maintain AZ Fingerprint Clearance Card
• Complete required compliance documentation and certifications
• Utilize Cerner charting system for patient documentation
• Adhere to business casual dress code requirements
• Complete required modules and training programs during orientation week
• Work 2 out of 3 major holidays as required
• Provide two professional references covering required timeframes
• Complete laboratory testing within specified timeframes
• Participate in quality and patient safety standards including medication administration protocols
Job Requirements:
- Active, unencumbered license in AZ
- Minimum of 1-7 years of relevant experience, depending on specialty
- Current Certifications: BLS and/or ACLS, plus any specialty-specific requirements
- Experience with EMR systems (Epic, Cerner, as applicable)
Other Required Documentation:
- Current Resume listing detailed work history (MM/YYYY format)
- Two current clinical references
- State-required professional license
- Physicial exam (completed within the past 12 months)
- TB skin test or Quantiferon Gold (within the past 12 months)
- Immunity Titers - MMR/Hep B/Varicella (declinations may be accepted)
- Current Tdap and Flu vaccinations
- Must pass background check and drug screening
