Position Summary:
VitalHire is seeking a skilled and compassionate PTA - Physical Therapist Assistant Rehab to join our growing clinician community. Key Responsibilities:
• Provide physical therapy assistant services in home health setting treating adults and geriatrics • Visit 4-6 patients per day maintaining 27-30 productivity points standard • Call physicians to request changes to Care Plan and obtain frequency/duration orders in collaboration with PT • Complete competent documentation during patient visits using provided documentation tools • Maintain timely communication and documentation as mandatory requirement • Provide care for patients with diagnoses including CHF, COPD, pneumonia, MS, ALS, diabetes, post-surgical conditions, joint replacements, fractures, and spinal conditions • Utilize skills in acute care, outpatient clinic, rehabilitation, and skilled nursing facility settings • Work with equipment including crutches, gait belts, prosthetics, walkers, and shoulder slings • Treat conditions such as hip fractures, post-op surgeries, cognitive disorders, Multiple Sclerosis, Parkinson's disease, stroke rehabilitation, and traumatic brain injuries • Float to other territories as needed • Work Tuesday-Saturday or Monday-Friday schedule as assigned • Complete 40 hours of orientation training • Maintain minimum caseload of 35 patients per week • Collaborate with interdisciplinary team including Physical Therapy, Interpretation Services, and Social Services • Obtain manager approval for overtime work and holiday coverage • Complete required compliance documentation and training modules • Maintain current BLS certification and valid state licensure • Provide own transportation with occasional travel radius up to 100 miles • Return all facility-provided documentation tools upon completion
Job Requirements:
- Active, unencumbered license in WA
- Minimum of 1-7 years of relevant experience, depending on specialty
- Current Certifications: BLS and/or ACLS, plus any specialty-specific requirements
- Experience with EMR systems (Epic, Cerner, as applicable)
Other Required Documentation:
- Current Resume listing detailed work history (MM/YYYY format)
- Two current clinical references
- State-required professional license
- Physicial exam (completed within the past 12 months)
- TB skin test or Quantiferon Gold (within the past 12 months)
- Immunity Titers - MMR/Hep B/Varicella (declinations may be accepted)
- Current Tdap and Flu vaccinations
- Must pass background check and drug screening
