Position Summary:
VitalHire is seeking a skilled and compassionate OT - Therapy Rehab to join our growing clinician community. This high-impact opportunity offers a 5x8 Days at a leading Rehab in Lacey, WA. Key Responsibilities:
• Provide occupational therapy services in home health setting • Conduct patient visits with 1:1 ratio, up to 6 patients per day • Complete OASIS assessments for Start of Care patients • Maintain 27-30 productivity points standard based on assigned territory • Document patient care using HomeCare HomeBase charting system • Provide timely communication and documentation as required • Maintain current BLS certification and NBCOT credentials • Hold valid WA state occupational therapy license • Utilize provided documentation tools and return equipment as required • Provide occupational therapy services to adult and geriatric patients • Travel within assigned territory with personal transportation • Complete compliance requirements and training within 2 weeks • Participate in on-call rotation as required • Submit time off requests at time of application • Complete required Hospice/Home Health Checklist • Maintain professional standards for home health care delivery
Job Requirements:
- Active, unencumbered license in WA
- Minimum of 1-7 years of relevant experience, depending on specialty
- Current Certifications: BLS and/or ACLS, plus any specialty-specific requirements
- Experience with EMR systems (Epic, Cerner, as applicable)
Other Required Documentation:
- Current Resume listing detailed work history (MM/YYYY format)
- Two current clinical references
- State-required professional license
- Physicial exam (completed within the past 12 months)
- TB skin test or Quantiferon Gold (within the past 12 months)
- Immunity Titers - MMR/Hep B/Varicella (declinations may be accepted)
- Current Tdap and Flu vaccinations
- Must pass background check and drug screening
