Position Summary:
VitalHire is seeking a skilled and compassionate OT - Therapy Rehab to join our growing clinician community. This high-impact opportunity offers a 5x8 Days at a leading SNF in Lacey, WA. Key Responsibilities:
• Provide occupational therapy services in home health setting for adult and geriatric patients
• Conduct patient visits with 1:1 ratio, up to 6 patients per day to meet 27-30 productivity points
• Complete OASIS documentation for Start of Care visits
• Utilize HomeCare HomeBase charting system for all documentation
• Maintain timely communication and documentation requirements
• Provide occupational therapy services including activities of daily living, biofeedback, cardiac rehabilitation, and various therapeutic modalities
• Perform specialized treatments for hand injuries, hip and knee replacements, prosthetics fitting, and splinting
• Travel within assigned territory with occasional radius up to 100 miles
• Maintain current BLS certification and NBCOT certification
• Hold valid Washington state occupational therapy license
• Provide personal transportation for patient visits
• Return all provided documentation tools and equipment
• Complete compliance documentation within 2-week timeframe
• Participate in on-call rotation as required
• Complete Hospice/Home Health Checklist
• Submit requested time off at time of application
Job Requirements:
- Active, unencumbered license in WA
- Minimum of 1-7 years of relevant experience, depending on specialty
- Current Certifications: BLS and/or ACLS, plus any specialty-specific requirements
- Experience with EMR systems (Epic, Cerner, as applicable)
Other Required Documentation:
- Current Resume listing detailed work history (MM/YYYY format)
- Two current clinical references
- State-required professional license
- Physicial exam (completed within the past 12 months)
- TB skin test or Quantiferon Gold (within the past 12 months)
- Immunity Titers - MMR/Hep B/Varicella (declinations may be accepted)
- Current Tdap and Flu vaccinations
- Must pass background check and drug screening
