Position Summary:
VitalHire is seeking a skilled and compassionate PT - Physical Therapist to join our growing clinician community. This high-impact opportunity offers a 5x8 Days at a leading SNF in Lacey, WA. Key Responsibilities:
• Provide physical therapy services in home health setting for adult and geriatric patients • Conduct patient visits with 1:1 ratios, seeing 4-6 patients per day • Achieve 27-30 productivity points standard based on assigned territory • Maintain current BLS certification and valid WA state license • Complete OASIS documentation for Start of Care patients • Use Home Care Home Base (HCHB) charting system for documentation • Provide timely communication and complete documentation requirements • Work 8-hour day shifts, including 4 weekdays and 1 weekend day per week • Travel to patient homes using personal transportation with average 30 miles/day driving • Navigate using iPhone and navigation apps to locate patients • Create and manage patient schedules independently • Use Outlook for email and calendar appointments • Participate in Teams meetings as required • Order PPE and therapy supplies via Medline online system • Treat various diagnoses including neuro, cardiac, respiratory, orthopedic, post-surgical, and oncology patients • Conduct wheelchair evaluations and equipment acquisitions when needed • Sync documentation daily and maintain accurate records • Demonstrate flexibility and adaptability in home health environment • Drive safely in various weather conditions including snow, rain, and on gravel roads • Complete 32 hours of orientation and required compliance documentation • Submit requested time off at time of application • Complete Hospice/Home Health Checklist as mandatory requirement
Job Requirements:
- Active, unencumbered license in WA
- Minimum of 1-7 years of relevant experience, depending on specialty
- Current Certifications: BLS and/or ACLS, plus any specialty-specific requirements
- Experience with EMR systems (Epic, Cerner, as applicable)
Other Required Documentation:
- Current Resume listing detailed work history (MM/YYYY format)
- Two current clinical references
- State-required professional license
- Physicial exam (completed within the past 12 months)
- TB skin test or Quantiferon Gold (within the past 12 months)
- Immunity Titers - MMR/Hep B/Varicella (declinations may be accepted)
- Current Tdap and Flu vaccinations
- Must pass background check and drug screening
