Position Summary:
VitalHire is seeking a skilled and compassionate PT - Physical Therapist to join our growing clinician community. This high-impact opportunity offers a 5x8 Days at a leading SNF in Lacey, WA. Key Responsibilities:
• Provide physical therapy services in home health setting to adult and geriatric patients • Complete 6 daily visits or minimum of 30 points weekly for productivity • Conduct OASIS evaluations and Start of Care documentation • Utilize HCHB (Homecare Homebase) system for documentation and scheduling • Travel within 60-mile radius territory to provide patient care services • Maintain timely documentation and communication with healthcare team • Treat patients with neuro, cardiac, respiratory, orthopedic, post-surgical, post-fall, and cancer diagnoses • Develop and implement home exercise programs for patients • Provide patient and family education on treatment plans • Work flexible schedule including 4 weekdays and 1 weekend day (Tuesday-Saturday or Sunday-Thursday plus 3 weekdays) • Maintain current BLS certification and valid state physical therapy license • Participate in on-call rotation as required • Use iPhone and navigation apps for patient visits and scheduling • Utilize Outlook for email and calendar appointments • Participate in Teams meetings as required • Order PPE and therapy supplies through Medline online system • Drive personal vehicle for patient visits with average 30 miles per day • Complete required orientation and annual training programs • Adhere to dress code requirements (scrubs, no jeans) • Complete daily documentation syncing and mobile Epic usage • Conduct wheelchair evaluations and equipment acquisitions when needed
Job Requirements:
- Active, unencumbered license in WA
- Minimum of 1-7 years of relevant experience, depending on specialty
- Current Certifications: BLS and/or ACLS, plus any specialty-specific requirements
- Experience with EMR systems (Epic, Cerner, as applicable)
Other Required Documentation:
- Current Resume listing detailed work history (MM/YYYY format)
- Two current clinical references
- State-required professional license
- Physicial exam (completed within the past 12 months)
- TB skin test or Quantiferon Gold (within the past 12 months)
- Immunity Titers - MMR/Hep B/Varicella (declinations may be accepted)
- Current Tdap and Flu vaccinations
- Must pass background check and drug screening
