Position Summary:
VitalHire is seeking a skilled and compassionate PT - Physical Therapist to join our growing clinician community. This high-impact opportunity offers a 5x8 Days at a leading SNF in Lacey, WA. Key Responsibilities:
• Provide physical therapy services in home health setting for adult and geriatric patients • Conduct patient visits with 1:1 ratios, visiting up to 6 patients per day • Achieve 27-30 productivity points standard based on assigned territory • Work 4 weekdays and 1 weekend day each week • Complete OASIS documentation for Start of Care visits • Maintain current BCLS certification and valid state licensure • Use Home Care Home Base charting system for documentation • Provide timely communication and documentation as required • Drive personal transportation to patient homes with average of 30 miles per day • Navigate up to 100-mile radius in various weather conditions including snow, rain and gravel roads • Use iPhone and navigation apps for patient scheduling and routing • Participate in on-call rotation as required • Use Outlook for email and calendar appointments • Participate in Teams meetings • Order PPE and therapy supplies via online platforms • Complete 32 hours of orientation training • Treat various diagnoses including neuro, cardiac, respiratory, orthopedic, post-surgical, and cancer patients • Perform wheelchair evaluations and equipment acquisition when needed • Maintain flexibility and adaptability in daily scheduling • Complete required compliance documentation and Hospice/Home Health checklist • Submit requested time off at time of application • Sync documentation daily using required systems
Job Requirements:
- Active, unencumbered license in WA
- Minimum of 1-7 years of relevant experience, depending on specialty
- Current Certifications: BLS and/or ACLS, plus any specialty-specific requirements
- Experience with EMR systems (Epic, Cerner, as applicable)
Other Required Documentation:
- Current Resume listing detailed work history (MM/YYYY format)
- Two current clinical references
- State-required professional license
- Physicial exam (completed within the past 12 months)
- TB skin test or Quantiferon Gold (within the past 12 months)
- Immunity Titers - MMR/Hep B/Varicella (declinations may be accepted)
- Current Tdap and Flu vaccinations
- Must pass background check and drug screening
