Position Summary:
VitalHire is seeking a skilled and compassionate PT - Physical Therapist to join our growing clinician community. This high-impact opportunity offers a 5x8 Days at a leading LTC in Lacey, WA. Key Responsibilities:
• Provide physical therapy services in home health setting treating adult and geriatric patients • Complete 6 daily visits or maintain minimum of 30 points weekly for productivity • Manage caseload of 5-7 patients per day on average • Perform OASIS assessments and start of care documentation as required • Utilize HCHB (Homecare Homebase) electronic medical record system for documentation • Provide timely communication and complete documentation requirements • Travel within 60-mile radius territory from hub office, occasionally further based on patient needs • Treat patients with various diagnoses including neuro, cardiac, respiratory, orthopedic, post-surgical, post-fall, and cancer conditions • Work with conditions such as cervicalgia, low back pain, post-operative surgeries, spinal conditions, joint replacements, cognitive disorders, Multiple Sclerosis, Parkinson's disease, and stroke rehabilitation • Develop and implement home exercise programs for patients • Provide patient and family education on treatment plans • Conduct wheelchair evaluations and equipment acquisition when needed • Maintain current BLS certification and state physical therapy license • Work 4 weekdays and 1 weekend day each week with 8-hour shifts • Use facility-provided iPad for documentation and patient management • Navigate using iPhone and GPS applications to locate patient homes • Create and manage patient schedules independently • Use Microsoft Outlook for email and calendar appointments • Participate in Teams meetings as required • Order physical therapy supplies and PPE through online systems • Drive personal vehicle with mileage reimbursement at IRS standard rate • Maintain ability to drive in various weather conditions on different road surfaces • Complete daily documentation synchronization requirements • Participate in on-call rotation as assigned • Complete required orientation hours and ongoing training requirements • Adhere to professional dress code requirements
Job Requirements:
- Active, unencumbered license in WA
- Minimum of 1-7 years of relevant experience, depending on specialty
- Current Certifications: BLS and/or ACLS, plus any specialty-specific requirements
- Experience with EMR systems (Epic, Cerner, as applicable)
Other Required Documentation:
- Current Resume listing detailed work history (MM/YYYY format)
- Two current clinical references
- State-required professional license
- Physicial exam (completed within the past 12 months)
- TB skin test or Quantiferon Gold (within the past 12 months)
- Immunity Titers - MMR/Hep B/Varicella (declinations may be accepted)
- Current Tdap and Flu vaccinations
- Must pass background check and drug screening
