Position Summary:
VitalHire is seeking a skilled and compassionate OT - Therapy Rehab to join our growing clinician community. This high-impact opportunity offers a 5x8 Days at a leading SNF in Lacey, WA. Key Responsibilities:
• Provide occupational therapy services in home health setting • Complete 6 daily visits or minimum of 30 points weekly for productivity • Maintain current BLS and NBCOT certifications • Utilize HCHB (Homecare Homebase) charting system for documentation • Provide care for geriatric patients including activities of daily living assessment • Travel within 60-mile radius territory for patient visits • Maintain timely documentation and communication requirements • Work flexible schedule with territory assignments • Perform OASIS assessments for Start of Care visits • Provide specialized therapy services including biofeedback, electrical stimulation, and TENS • Assess and treat various conditions including arthritis, hand injuries, hip and knee replacements • Fit and train patients on prosthetics and splinting devices • Collaborate with interdisciplinary home health team • Maintain valid state occupational therapy license • Complete required compliance documentation and training programs • Adhere to facility dress code requirements wearing scrubs • Submit requested time off at time of application • Complete Hospice/Home Health Checklist as required
Job Requirements:
- Active, unencumbered license in WA
- Minimum of 1-7 years of relevant experience, depending on specialty
- Current Certifications: BLS and/or ACLS, plus any specialty-specific requirements
- Experience with EMR systems (Epic, Cerner, as applicable)
Other Required Documentation:
- Current Resume listing detailed work history (MM/YYYY format)
- Two current clinical references
- State-required professional license
- Physicial exam (completed within the past 12 months)
- TB skin test or Quantiferon Gold (within the past 12 months)
- Immunity Titers - MMR/Hep B/Varicella (declinations may be accepted)
- Current Tdap and Flu vaccinations
- Must pass background check and drug screening
