Position Summary:
VitalHire is seeking a skilled and compassionate RN - Case Management to join our growing clinician community. Key Responsibilities:
• Perform case management services for acute care hospital patients with focus on care coordination and discharge planning
• Assess patients using InterQual admission criteria, particularly in emergency department settings
• Coordinate care for diverse patient populations including adolescents, adults, and geriatrics
• Develop and participate in treatment plans for patients
• Organize and participate in family meetings
• Provide discharge planning services including arrangement of DME, SNF placement, and ambulance transport independently
• Coordinate care for complex patient populations including those with drug/alcohol abuse issues, homelessness, mental health history or crisis situations
• Manage advance directives, POLST documentation, and protective services processes
• Work with HMOs, IPAs, and other insurance entities
• Utilize Epic charting system for computerized documentation
• Maintain proficiency in Microsoft applications
• Collaborate with interdisciplinary support teams including Social Services and Transportation
• Manage patient caseload of approximately 20 patients during day shifts
• Work every other weekend as required
• Commit to working 3 of the 5 major holidays (Thanksgiving, Christmas Eve, Christmas Day, New Year's Eve, New Year's Day)
• Float between facilities within the state as required
• Complete required pre-start and annual training modules
• Utilize facility-provided communication devices when required for assignment duties
• Provide minimum of 2 supervisory references covering 1 year of employment within comparable specialty/setting
• Complete skills checklist and required compliance documentation
Job Requirements:
- Active, unencumbered license in CA
- Minimum of 1-7 years of relevant experience, depending on specialty
- Current Certifications: BLS and/or ACLS, plus any specialty-specific requirements
- Experience with EMR systems (Epic, Cerner, as applicable)
Other Required Documentation:
- Current Resume listing detailed work history (MM/YYYY format)
- Two current clinical references
- State-required professional license
- Physicial exam (completed within the past 12 months)
- TB skin test or Quantiferon Gold (within the past 12 months)
- Immunity Titers - MMR/Hep B/Varicella (declinations may be accepted)
- Current Tdap and Flu vaccinations
- Must pass background check and drug screening
